复发性阿弗他溃疡对患者生活质量影响的初步研究

摘　　要：目的通过口腔健康影响程度量表、复发性阿弗他溃疡(recurrent aphthous ulcer,RAU)患者病历、焦虑自测量表(self-rating anxiety scale,SAS)和抑郁自测量表(self-rating depression scale,SDS)研究复发性阿弗他溃疡对患者生活质量的影响。方法采用口腔健康影响程度量表(OHIP-14)中文版和焦虑、抑郁自测量表选择90例复发性阿弗他溃疡患者进行问卷调查,与100名健康人群问卷进行比较;同时记录RAU患者病历情况和视觉模拟评分法(VAS)测得疼痛分值,针对溃疡大小、数目、面积和出现部位计算量表得分并归类,采用SPSS25.0软件包对数据进行统计学分析。结果RAU患者的口腔健康状况较健康人群较差,并且在抑郁自测方面有显著差异,RAU患者OHIP-14总分值与VAS分值、SDS分值、SAS分值和溃疡个数之间呈正相关性。溃疡出现部位与VAS没有显著差异。结论复发性阿弗他溃疡对患者抑郁情绪有一定的相关性;溃疡个数越多以及由RAU引起的焦虑抑郁情绪越重,患者的口腔健康相关生活质量(oral health related quality of life,OHRQOL)越差。Objective The effect of recurrent aphthous ulcer on the quality of life was studied by the oral health impact scale,the medical records,the anxiety self-test scale and the depression self-test scale.Methods The effect of recurrent aphthous ulcer on the quality of life was studied with scales.The patients with recurrent aphthous ulcer were surveyed with OHIP-14 Chinese and anxiety and depression self-test,and their medical records and visual analog pain(VAS)were recorded,which were compared with the data of the general population in view of the ulcer size,number,area and position.SPSS 25.0 software package was used for statistical analysis.Results The oral health status of RAU was worse than that of healthy people,and there was a significant difference in the self-assessment of depression.There was a positive correlation between the total score of OHIP-14 and VAS,SDS,SAS and the number of ulcer.No significant difference was found between the location of ulcer and VAS.Conclusion Recurrent aphthous ulcer has certain influence on depression.The more the number of ulcers,the more severe the anxiety and depression caused by RAU,the worse the OHRQOL of patients,and there was no relationship between ulcer pain and the ulcer site.
