摘  要:目的应用锥形束CT(cone beam computed tomography,CBCT)测量下颌单管型切牙在不同横断面下根管壁的厚度,为根管治疗、桩道预备等临床操作提供参考。方法收集2018年12月至2020年12月就诊的170例患者632颗下颌单管型切牙的CBCT影像资料。测量其唇、舌、近中、远中根管壁在釉牙骨质界(cementoenamel junction,CEJ)、根颈1/3段与根中1/3段交界面(Interface between cervical third and middle third,CM)、根中1/3段与根尖1/3段交界面(interface between apical third and middle third,AM)处的厚度,对测量的数据进行分析。结果在CEJ处,近中壁与远中壁厚度差异无统计学意义(P>0.05),其余各壁间均有差异(P<0.05),舌壁>唇壁>近中、远中壁;在CM处,唇壁与舌壁、近中壁与远中壁厚度差异无统计学意义(P>0.05),唇、舌壁与近、远中壁间均有差异(P<0.05),唇、舌侧壁>近中、远中壁;在AM处,近中壁与远中壁厚度差异无统计学意义(P>0.05),其余各壁间均有差异(P<0.05),唇壁>舌壁>近中、远中壁。结论CBCT影像学检查能够三维反映下颌单管型切牙的解剖形态,在CEJ、CM及AM平面,根管近中、远中、唇、舌等壁的厚度存在不同的差异和特点,研究结果对于临床上进行下颌单管型切牙的根管治疗、桩道预备等临床操作中防止侧穿提供了重要参考。Objective In order to guide clinical operations such as root canal therapy and post preparation.,cone beam computed tomography(CBCT)was used to measure the thickness of the root canal walls under different cross sections of the mandibular single-canal incisors.Methods CBCT imaging data of 632 mandibular single-canal incisors of 170 patients from 2018.12 to 2020.12 was collected.The thickness of labial,lingual,mesial and distal root canal wall at the cementoenamel junction(CEJ),the interface between the cervical third and the middle third of canal(CM),and the interface between the middle third and the apical third of canal(AM)was measured,and the measured data was analyzed.Results At CEJ section,there was no significant difference in root canal wall thickness between mesial and distal root canal(P>0.05),but there were differences between the other walls(P<0.05)and the comparison showed that lingual wall>labial wall>mesial or distal root canal wall.At CM section,there was no significant difference in the thickness of labial and lingual root canal wall(P>0.05),as well as medial and distal root canal wall(P>0.05)but there was significant difference between labial,lingual wall and mesial,distal root canal wall(P<0.05)and the comparison showed that labial or lingual wall>mesial or distal wall.At AM section,there was no significant difference in root canal wall thickness between mesial and distal root canal(P>0.05),but there were differences between the other walls(P<0.05)and the comparison showed that labial wall>lingual wall>mesial or distal root canal wall.Conclusion CBCT can reflect the anatomical morphology of mandibular single-canal incisors in three dimensions.There are different differences and characteristics in the thickness of labial,lingual,mesial and distal walls of root canals in CEJ,CM and AM sections.The results provide an important reference for the prevention of root perforation in clinical operations such as root canal treatment and post preparation of mandibular single-canal incisors.
