摘 要:目的基于常规磁共振脉冲序列,探讨子宫颈癌(cervical cancer,CC)磁共振背景信号抑制弥散加权成像(magnetic resonance diffusion weighted imaging with background suppression,DWIBS)影像学特征,并进一步分析CC的ADC值与血管内皮生长因子(vascular endothelial growth factor,VEGF)、微血管密度之间的关系。方法选取符合纳入和排除标准的CC患者45例(观察组)及经证实的无妇科病变的患者40例(对照组)的MRI资料和临床资料进行回顾性分析,测量正常宫颈各层以及CC的肿瘤实质、肿瘤边缘和肿瘤周边正常肌层的信号强度、ADC值,并检测肿瘤VEGF、CD34表达情况。结果肿瘤实质与正常宫颈粘膜皱襞的信号强度、ADC值比较差异有统计学意义(P<0.01),与肿瘤边缘的信号强度及ADC值差异无统计学意义(P>0.05);CC的实质和肿瘤边缘与肿瘤旁正常肌层比较差异有统计学意义(P<0.01);CC高分化和中分化肿瘤ADC值比较差异有统计学意义(P<0.05);CC的ADC值与VEGF表达、肿瘤微血管面积具有负相关性(r=-0.72、r=-0.59,P<0.05)。结论DWIBS对CC的临床诊断及分期有重要临床价值。CC的DWIBS信号强度及ADC值可以在一定程度上提示肿瘤组织的病理学信息。Objective To investigate the imaging characteristics of cancer of cervix(CC)by magnetic resonance weighted imaging with background suppression(DWIBS).The relationship between the ADC value of CC,vascular endothelial growth factor(VEGF)and microvascular density was further analyzed.Methods The pelivic magnetic imaging and dinica data of CG patiets(45 cases)and the confirmed non-gynecological diseases in patients(40 cases)who met the inclusion criteria and exclusion criteria were collected and analyzed retrospectively.The signal intensity and ADC values of the normal cervical layers,the tumor parenchyma of CC,the tumor margin and the normal muscle layer around the tumor were measured,and the expressions of VEGF and CD34 in the tumor were detected.Results The signal intensity and ADC values of the parenchyma of CC were statistically significant compared with those of the normal cervical plica(P<0.01),but there was no statistically significant difference between the signal intensity and ADC values at the peritumor edge(P>0.05).The difference between CC parenchyma and tumor margins was statistically significant(P<0.01).The signal intensity and ADC values of the normal muscle layer around the tumor were statistically significant compared with parenchyma of CC and the tumor edge(P<0.01).There was a statistical difference in ADC values between high-differentiated and medium-differentiated CC tumors(P<0.05).The ADC value in the parenchyma of CC was negatively correlated with VEGF expression and tumor microvascular area(r=-0.72,r=-0.59,P<0.05).Conclusion DWIBS has important clinical value in the clinical diagnosis and staging of CC.The DWIBS signal strength and ADC value of CC can indicate the pathological information of tumor tissue to a certain extent.
