摘 要:目的研究SPECT/CT探测颅内肿瘤125I粒子植入放射浓聚计数值与剂量关系,探讨颅内肿瘤粒子植入后剂量可视化的可行性。方法在CT引导下应用模板辅助穿刺颅内肿瘤模型。所有植入针按照术前计划在肿瘤内呈锥形分布,CT扫描植入针位置无误后按照术前计划植入2.035×10^(7) Bq粒子25颗。然后将术后头颅模型进行SPECT/CT扫描及图像融合。CT图像传入计算机治疗计划系统(TPS)行验证计划。在肿瘤最大层面分别沿着和垂直中间植入针方向测量40 Gy、60 Gy、80 Gy、100 Gy、140 Gy等剂量线包绕区域的长径。在SPECT/CT融合图像上找出对应剂量(D)位置的放射性浓聚计数值(x),应用SPSS18.0软件比较沿着针道方向和垂直针道方向两组计算值有无差异,并计算计数值与剂量的关系,得出关系公式。结果沿着针道方向与TPS中40 Gy、60 Gy、80 Gy、100 Gy、140 Gy等剂量线对应的放射性浓聚计数值分别为38.6±5.7、65.2±5.4、87±4.5、110±5.2、132.8±7.3;垂直针道方向与相应剂量对应的计数值分别81.7±22.8、178.2±49.8、247.7±65.8、289.7±67.2、358.7±74.5,两组相比差异有统计学意义,P<0.001。针道方向上计数值(x)与剂量(D)关系为指数关系,方程为:D=25.787×1.013^(X)。结论SPECT/CT显像可使颅内肿瘤125I粒子植入周围剂量可视化,放射性计数值与粒子剂量存在指数关系,为研发粒子植入颅内肿瘤术后剂量验证新方法奠定了理论基础。Objective To study the relationship between dose and radioactive counts of 125I seeds in brain tumor detected by SPECT/CT and to observe the possibility of dose visualization.Methods Puncture the brain tumor phantom with a template underwent the guidance of CT scan.All the needles distributed like a cone in tumor phantom according to preplan.Twenty-five seeds with the activity of 2.035×10^(7) Bq were implanted after CT confirmed the needles’location.The phantoms were scanned by SPECT/CT,and the SPECT images and CT images were fused.Post-implant dose verification was performed by treatment planning system(TPS)with CT images.The long diameters of 40 Gy,60 Gy,80 Gy,100 Gy,and 140 Gy isodose lines were measured along and perpendicular the middle needle in the middle slice of the tumor respectively.The radioactive counts values(x)correspond with a certain dose(D)were determined.The count values along and perpendicular the middle needle were recorded and compared with SPSS18.0 software.The dose was plotted against count values,and the corresponding curve equations were ob-tained with linear regression.Results The correspond count values along the needle were 38.6±5.7,65.2±5.4,87.0±4.5,110.0±5.2,and 132.8±7.3.The correspond count values perpendicular the needle were 81.7±22.8,178.2±49.8,247.7±65.8,289.7±67.2,and 358.7±74.5.The difference was statistically significant in the two groups,P<0.001.There was an exponent relationship between radioactive counts values(x)along the needle and dose(D).The equation was D=25.787×1.013^(X).Conclusion Dose visualization of 125I seed in brain tumor can be achieved by SPECT/CT.There is an exponent relationship between radioactive counts values and dose,which makes the foundation of developing a new dose verification method for brain tumor seed implantation.
