锥颅穿刺抽吸引流术治疗脑干出血的临床价值

摘　　要：目的探讨锥颅穿刺抽吸引流术治疗脑干出血的临床效果。方法选取2010年5月至2020年12月我院收治的60例脑干出血患者,随机分为两组各30例。手术组给予锥颅穿刺抽吸引流术治疗,保守组给予药物保守治疗。比较两组的血肿清除量、意识障碍程度(GCS评分)、神经缺损程度(NIHSS评分)以及预后(m RS分级)。结果治疗后,两组的GCS评分均显著高于治疗前,NIHSS评分均显著低于治疗前(P<0.05);手术组的GCS评分显著高于保守组,NIHSS评分显著低于保守组(P<0.05)。手术组的血肿清除量显著高于保守组,mRS分级显著低于保守组(P<0.05)。结论锥颅穿刺抽吸引流术治疗脑干出血患者可有效清除血肿,减轻患者意识障碍程度,促进患者神经功能恢复,改善患者预后。Objective To explore the clinical effect of aspiration and drainage through drilling cranium puncture in the treatment of brainstem hemorrhage.Methods Sixty patients with brainstem hemorrhage admitted to our hospital from May 2010 to December 2020 were selected and randomly divided into two groups,with 30 cases in each group.The surgical group was treated with aspiration and drainage through drilling cranium puncture,and the conservative group was treated with drug conservative therapy.The hematoma clearance volume,degree of consciousness disorder(GCS score),degree of neurological deficit(NIHSS score)and prognosis(mRS grade)were compared between the two groups.Results After treatment,the GCS scores of the two groups were significantly higher than those before treatment,and the NIHSS scores were significantly lower than those before treatment(P<0.05);The GCS score in the surgical group was significantly higher than that in the conservative group,and the NIHSS score was significantly lower than that in the conservative group(P<0.05).The hematoma clearance volume in the surgical group was significantly higher than that in the conservative group,and the mRS grade was significantly lower than that in the conservative group(P<0.05).Conclusions Aspiration and drainage through drilling cranium puncture in the treatment of patients with brainstem hemorrhage can effectively remove hematoma,reduce the degree of consciousness disorder,promote the recovery of neurological function and improve the prognosis of patients.
