摘 要:目的探讨积极心理干预在抑郁障碍伴非自杀性自伤行为青少年的应用效果。方法选取2020年2-12月临床心理科收治的76例抑郁障碍伴非自杀性自伤行为青少年患者为研究对象,采用随机数字表法将其分为对照组与研究组,各38例;2组患者均口服帕罗西汀,对照组实施常规护理,研究组实施积极心理干预,干预3个月后,采用汉密尔顿抑郁量表(Hamilton depression scale,HAMD)、自我伤害问卷及多伦多述情障碍量表(toronto alexithymia scale,TAS)对2组进行评估。结果2组HAMD得分在时间效应、组间效应及交互效应比较,差异有统计学意义(F_(时间)=14.320,P<0.001;F_(组间)=7.680,P=0.020;F_(交互)=11.420,P<0.001);干预后,研究组自我伤害问卷得分低于对照组(t=6.644,P<0.001)。TAS量表总分及描述情感能力、识别情绪困难、外向型思维维度得分均低于对照组(t=4.020,P<0.001;t=2.681,P=0.009;t=3.480,P<0.001;t=2.611,P=0.010)。结论积极心理干预在抑郁障碍伴非自杀性自伤行为青少年可降低抑郁程度,减轻自我伤害行为,提升情绪控制能力。Objective To explore the application effect of positive psychological intervention(PPI)on depressive adolescents with non-suicidal self-injury(NSSI).Methods A total of 76 adolescent patients with depression and NSSI treated in department of clinical psychology between February and December 2020 were randomly divided into a control group and a research group,each of 38,according to a random number table.In addition to taking paroxetine orally,the control group was given routine care,while the research group was provided with PPI for 3 months.Before and after the intervention,both groups were evaluated using the Hamilton depression scale(HAMD),self-injury questionnaire and Toronto alexithymia scale(TAS).Results The average HAMD score of both groups after 3-month intervention was significantly lower than before the intervention,with that of the research group significantly lower than the latter(F_(time)=14.320,P<0.001;F_(intergroup)=7.680,P=0.020;F_(interaction)=11.420,P<0.001).The average self-injury questionnaire scores of the research group after 3 months of intervention were significantly lower than that of the control group(t=6.644,P<0.001).After the intervention,the average total TAS score,together with the average score of the dimension of the ability to describe emotions,difficulty in recognizing emotions and extroverted thinking of the research group was significantly lower than the control group(t=4.020,P<0.001;t=2.681,P=0.009;t=3.480,P<0.001;t=2.611,P=0.010).Conclusion Positive psychological intervention can relieve depression,alleviate self-injury and improve emotional control of adolescents with depressive disorder and non-suicidal self-injury.
