摘　　要：目的评价Gluma脱敏剂、Nd:YAG激光和Er:YAG激光单独及联合治疗非手术牙周治疗引起的牙本质过敏症(DH)的疗效。方法选择60例经非手术牙周治疗后发生DH的牙周炎患者,共计180颗患牙。将患牙随机分为6组,每组30颗,其中A组:空白对照组;B组:Gluma脱敏剂组;C组:Nd:YAG激光组;D组:Er:YAG激光组;E组:Gluma脱敏剂+Nd:YAG激光组;F组:Gluma脱敏剂+Nd:YAG激光组。用视觉模拟评分法(visual analogue scale,VAS)评价牙齿在治疗前、治疗后即刻、治疗后1周、1个月、3个月对温度及机械刺激敏感程度的变化。结果除A组(对照组)外,各组治疗后即刻和治疗后1周的温度刺激和机械刺激VAS值均显著低于治疗前(P<0.05)。治疗后1个月,除A、B组外,其余4组的温度刺激和机械刺激的VAS值与治疗前相比有显著性差异(P<0.05)。在治疗后3个月,各组的VAS值均低于治疗前。在治疗后各时间点,E、F联合治疗组VAS值均显著低于其他4组,然而,E组和F组之间没有显著差异。结论 Nd:YAG激光、Er:YAG激光和Gluma脱敏剂联合应用可显著减轻非手术牙周治疗引起的DH症状,近期和远期疗效显著。Objective This clinical investigation aimed to evaluate and compare the changes of dentin hypersensitivity(DH)caused by non-surgical periodontal therapy after Gluma desensitizer,Nd:YAG laser,Er:YAG laser and their combination treatment.Methods Subjects with periodontal diseases who had finished non-surgical periodontal therapy were included in this study.180 lesions from 60 patients were randomly divided into 6 groups,including A:control group(n=30);C:Nd:YAG laser group(n=30);D:Er:YAG laser group(n=30);E:Gluma +Nd:YAG laser group(n=30);F:Gluma +Nd:YAG laser group(n=30).Visual analogue scale(VAS)was used to assess the level of sensitivity of each volunteer at baseline,immediately,1 week,1 month and 3 months after the treatment.All analyses were performed separately for cold air and tactile stimulus.Results the VAS of postoperative and 1-week response in all groups were significantly decreased (P<0.05) than baseline except the control group.At 1 months after treatment,the VAS of the Nd:YAG laser group,Er:YAG laser group,Gluma+Nd:YAG laser group,Gluma+Nd:YAG laser group were significantly lower than control group and Gluma group.The Gluma+Nd:YAG laser group and Gluma+Nd:YAG laser group obtained the most effective results than any other groupsatanytimeaftertreatment.There was no significant difference between Gluma+Nd:YAG laser group and Gluma+Nd:YAG laser group.Conclusion Combining Gluma and Nd:YAG or Er:YAG can effectively reduce the sensitivity of DH caused by non-surgical periodontal therapy.
