摘  要:目的:比较增强免疫组化(Ventana immunohistochemistry,Ventana IHC)检测非小细胞肺癌(non-small cell lung cancer,NSCLC)C-ros原癌基因1-受体酪氨酸激酶(C-ros oncogene 1 receptor tyrosine kinase,ROS1)蛋白表达和二代测序(next-generation sequencing,NGS)技术检测ROS1基因融合突变的一致性。方法:应用NGS技术在DNA层面检测966例NSCLC患者标本ROS1基因融合突变情况,同时应用Ventana IHC检测其中732例标本ROS1蛋白的表达情况。结果:NGS检测结果显示966例NSCLC患者的ROS1基因融合突变率为1.0%(10/966),阳性样本病理类型为肺腺癌。Ventana IHC检测结果显示ROS1蛋白表达阳性率为17.6%(129/732),阳性样本病理类型主要为肺腺癌。Ventana IHC和NGS两种方法检测结果的一致性为83.6%(612/732),kappa=0.110(P<0.001)。两种方法检测ROS1基因差异具有统计学意义(P<0.001)。结论:作为ROS1基因的检测方法,Ventana IHC法比NGS法更灵敏,两种方法检测结果的一致性较差,两者差异具有统计学意义。Ventana IHC筛查出ROS1阳性样本,可采用NGS进行验证。Objective:To compare the consistency of enhanced immunohistochemistry(Ventana IHC)and next-generation sequencing(NGS)for dection of ROS1-positive in non-small cell lung cancer(NSCLC).Methods:NGS was used to analyze the ROS1 gene rearrangement in 966 patients with NSCLC,and ventana IHC was used to analyze the expression of ROS1 protein in 732 patients with NSCLC.Results:The NGS method showed that the rate of ROS1 gene fusion in our NSCLC samples was 1.0%(10/966).The pathological type of the positive sample was lung adenocarcinoma.The Ventana IHC method showed that the positive rate of ROS1 protein expression was 17.6%(129/732).The pathological type of positive samples was mainly lung adenocarcinoma.The consistency between NGS and Ventana IHC was 83.6%,kappa=0.110(P<0.001).The difference between the two methods to detect ROS1 gene was statistically significant(P<0.001).Conclusion:As the detection method of ROS1,Ventana IHC is more sensitive than NGS.The results are low consistency in ROS1-positive detection,and the difference between them is statistically significant.ROS1 positive samples can be screened by Ventana IHC,which can be verified by NGS.
