双歧杆菌联合非甾体抗炎药物对广泛侵袭性牙周炎患者远期牙齿缺失的影响

摘　　要：目的探讨双歧杆菌联合非甾体抗炎药物对广泛侵袭性牙周炎患者远期牙齿缺失的影响。方法选取2018年4月~2019年4月共收治的60例广泛侵袭性牙周炎患者作为研究对象,将所有患者依照1:1的比例随机分为观察组与对照组,每组30例。对照组患者进行根面平整术、龈下刮治、龈上洁治、口服甲硝唑和阿莫西林相关常规治疗,观察组在常规治疗基础上应用双歧杆菌含服联合布洛芬缓释凝胶外用,对比两组患者治疗前后CAL、PD、BI、PLI水平变化,对比两组患者治疗前后炎症因子表达水平,对比两组患者治疗后1个月、半年、1年以及2年的牙齿数目。结果两组患者治疗前CAL、PD、BI、PLI表达水平对比无明显差异,治疗后两组患者CAL、PD、BI、PLI表达水平均降低,且观察组低于对照组,组间对比,差异具有统计学意义(P<0.05);两组患者治疗前IL-6、IL-1、TNF-α相关炎症因子水平对比无明显差异(P>0.05),治疗后两组患者IL-6、IL-1、TNF-α相关炎症因子水平均降低,且观察组低于对照组,组间对比,差异具有统计学意义(P<0.05);两组患者治疗前、治疗半年后、治疗1年后牙齿数目对比无明显差异(P>0.05),两组患者治疗2年后牙齿缺失数目减少,观察组缺失数少于对照组,组间对比,差异具有统计学意义(P<0.05)。结论对广泛侵袭性牙周炎患者在常规治疗基础上应用双歧杆菌联合非甾体抗炎药物治疗能够提升治疗效果,提升患者的牙周健康程度,降低短期炎症因子表达,减少患者远期牙齿缺失率。Objective To investigate the effect of bifidobacterium combined with non steroidal anti-inflammatory drugs on long-term tooth loss in patients with extensive aggressive periodontitis.Methods 60 patients with extensive aggressive periodontitis in our hospital from April 2018 to April 2019 were selected as the research objects,and all patients were randomly divided into the observation group and the control group according to the ratio of 1:1,with 30 cases in each group.Patients in the control group were treated with root planing,subgingival scaling,gingival scaling,oral administration of metronidazole and amoxicillin.The observation group was treated with bifidobacterium plus ibuprofen sustained release gel on the basis of conventional treatment.The levels of CAL,PD,BI and PLI were compared between the two groups before and after treatment.The expression levels of inflammatory factors before and after treatment,the number of teeth before treatment,half a year after treatment,1 year after treatment and 2 years after treatment were compared between the two groups.Results There was no significant difference in the expression levels of CAL,PD,BI and PLI between the two groups before treatment.After treatment,the expression levels of CAL,PD,Bi and PLI in the two groups were decreased,and the observation group was lower than the control group,the difference was statistically significant(P<0.05);There was no significant difference in the levels of IL-6,IL-1,and TNF-αrelated inflammatory factors between the two groups before treatment(P>0.05),and the levels of IL-6,IL-1,and TNF-αrelated inflammatory factors in the two groups after treatment all decreased,and the observation group was lower than the control group,the difference between the groups was statistically significant(P<0.05);There was no significant difference in the number of teeth between the two groups before treatment,half a year after treatment and one year after treatment(P>0.05).The number of teeth in the observation group was higher than that in the cont
