Ⅱ类错[牙合]生长型与牙颌补偿的相关性研究

摘　　要：目的观察骨性Ⅱ类错[牙合]患者生长型与牙颌补偿间的相关性。方法选取未经治疗成人骨性Ⅱ类错[牙合]患者86例。通过X线头影测量的方法对骨性Ⅱ类错合矢状向及垂直向不调程度同上下颌中切牙倾斜程度与[牙合]平面及下颌平面倾斜角度做相关性分析。选择上齿槽座点-鼻根点-下齿槽座点夹角(ANB角)与前颅底平面与上中切牙长轴的交角(SN/U1角)、下中切牙长轴与下颌平面交角(L1/MP角)、前颅底平面与[牙合]平面交角(SN/OP角)、前颅底平面与下颌平面交角(SN/MP角),行pearson相关性分析研究。结果ANB和SN/MP的测量结果在高角组、均角组和低角组三组间呈递减趋势(P<0.05),低角组L1/MP高于高角组和均角组(P<0.05),而均角组和低角组SN/OP、SN/MP低于高角组(P<0.05)。ANB与U1/SN呈负相关(r=-0.309,P<0.05),与SN/OP、SN/MP呈正相关(r=0.305、0.359,P<0.05)。结论骨性Ⅱ类错合的垂直生长型与矢状向不调程度有相关性,而颌骨矢状向不调越严重,下颌平面代偿性向下向后旋转,上切牙代偿性舌倾越多,[牙合]平面角代偿性增加越多。Objective To investigate the correlation between compensatory mechanism and facial types in skeletalⅡmalocclusion.Methods 86 adult patients of skeletalⅡmalocclusion were selected.The lateral cephalometric X-ray films before treatment in different vertical facial skeletal types were measured and compared.Then Pearson correlation and multiply regressive analyses were performed between the angles ANB and SN/U1,L1/MP,SN/OP,SN/MP.Results The measurement results of ANB and SN/MP showed a decreasing trend among the high angle group,the average angle group and the low angle group(P<0.05),and the L1/MP of the low angle group was higher than that of the high angle group and the average angle group(P<0.05),while the SN/OP and SN/MP of the equal angle group and the low angle group were lower than those of the high angle group(P<0.05).ANB was negatively correlated with U1/SN(r=-0.309,P<0.05),and positively correlated with SN/OP and SN/MP(r=0.305,0.359,P<0.05).Conclusions The sagittal types and dental compensatory mechanism can be affected by the developments of vertical skeletal types.As the skeletal discrepancy becomes worse,the maxillary inscisors inclines more palatally,the mandibular plane make a down backward rotation,and the occlusal plane continues precipitous.
