加味生脉饮对DTC患者术后^(131)I治疗时唾液腺保护的临床观察

摘　　要：目的观察加味生脉饮辅助维生素C对分化型甲状腺癌(DTC)患者术后清甲治疗时对唾液腺的保护作用。方法按随机数字表法,将86例分化型甲状腺癌患者进行甲状腺切除术后分为对照组和观察组,每组43例。对照组服碘后口服醋酸泼尼松,观察组在对照组治疗的基础上,服碘后联合加味生脉饮辅助治疗;治疗后,比较2组患者临床症状积分、生命质量评分、唾液腺功能指标(UR30、EP、ER、EF、MAR、MSR)、涎腺炎发生率。结果治疗后,观察组临床症状积分,生命质量积分高于对照组,差异有统计学意义(P<0.05);治疗前,对照组与观察组腮腺、颌下腺MAR、MSR比较差异无统计学意义(P>0.05),治疗后,2组患者腮腺MAR、MSR的P值分别为0.042、0.038,颌下腺的MAR、MSR的P值分别为0.041、0.029,差异有统计学意义(P<0.05);对照组与观察组左右腮腺、左右颌下腺的UR30、EF、EP服碘治疗后急剧下降,观察组下降幅度小于对照组;对照组、观察组1年期慢性涎腺炎发生率分别为27.91%、11.63%,差异有统计学意义(P<0.01)。结论加味生脉饮可在DTC患者术后采用服碘进行清甲治疗时保护唾液腺。Objective To observe the protective effect of Jiawei Shengmai decoction supplemented with vitamin C on salivary glands of patients with DTC during postoperative ablative treatment.Methods According to the random number table,86 patients with differentiated thyroid cancer who underwent thyroidectomy were divided into control group and observation group,with 43 cases in each group.The control group took iodine orally with prednisone acetate,and the observation group was treated with iodine combined supplemented with Jiawei Shengmai decoction as adjuvant therapy on the basis of the treatment of the control group.After treatment,clinical symptom scores,quality of life scores,and salivary gland function indexes(UR30,EP,ER,EF,MAR,MSR),and the incidence of sialadenitis were compared between the two groups.Results After treatment,the clinical symptom score and quality of life score of the observation group were significantly higher than those of the control group(P<0.05).Before treatment,there were no significant differences in MAR and MSR of parotid gland and submandibular gland between the two groups(P>0.05).After treatment,the P values of MAR and MSR of the parotid glands in the two groups were 0.042 and 0.038,respectively,and the P values of MAR and MSR of the submandibular glands were 0.041 and 0.029,respectively,with significant differences(P<0.05).The UR30,EF,EP of the left and right parotid glands in both groups were decreased sharply after iodine treatment,and the decrease degree in observation group was less than that in control group,and the incidence rates of chronic sialadenitis in control group and observation group were 27.91%and 11.63%,respectively,with significant differences(P<0.01).Conclusion Jiawei Shengmai decoction can protect the salivary glands of patients with DTC treated by iodine after operation.
