摘 要:目的探讨基于罗杰斯整体人理论的护理干预对围术期患儿焦虑的影响。方法选取2021年1—5月河南省洛阳正骨医院小儿骨伤科收治的80例患儿,采用随机数字表法分为对照组和观察组,每组各40例。对照组采用常规护理干预,观察组在对照组基础上,以罗杰斯整体人理论为框架给予共振性、螺旋性、整体性3个方面的围术期干预。比较两组患儿围术期的心率、平均动脉压,以及儿童情绪适应量表学前版、改良耶鲁术前焦虑量表、诱导期合作量表、儿童苏醒期躁动评分。结果观察组儿童情绪适应量表学前版在术前、随访时的评分均高于对照组(P<0.05);随访时,对照组患儿该量表评分低于术前(P<0.05),而观察组患儿该量表评分高于术前(P<0.05);观察组改良耶鲁术前焦虑量表、诱导期合作量表、儿童苏醒期躁动评分均低于对照组(P<0.05)。观察组患儿心率、平均动脉压随着时间的变化而变化,组间和时间的交互作用均有统计学意义(P<0.05)。结论基于罗杰斯整体人理论的围术期护理干预可改善患儿的焦虑情绪,提高患儿面对困难及陌生环境的调节能力。Objective To explore the effect of care based on Rogers’s science of unitary human beings on perioperative anxiety in children.Methods A total of 80 children admitted to the Pediatric Orthopaedics & Traumatology Department, Luoyang Orthopedic-Traumatologial Hospital of Henan Province, from January to May 2021 were selected. They were divided into the control group(n=40) and the observation group(n=40) by using a random number table. The control group was given routine care, while the observation group, on this basis, was given perioperative intervention from 3 aspects of resonance, helicity and integrity based on Rogers’s science of unitary human beings. Heart rate, mean arterial pressure, the score of Children’s Emotional Adjustment Scale-Preschool Version(CEAS-P), the score of modified Yale Preoperative Anxiety Scale(m-YPAS), the score of Induction Compliance Checklist(ICC) and the score of Pediatric Anesthesia Emergence Delirium(PAED) were compared between the two groups.Results The CEAS-P scores of the children in the observation group were higher than those in the control group before operations and during follow-up(P<0.05);the CEAS-P scores of the control group after follow-up were lower than the scores before operations(P<0.05), and the CEAS-P scores of the observation group after follow-up were higher than the scores before operations(P<0.05);the m-YPAS scores, ICC scores and PAED scores of the observation group were lower than those in the control group(P<0.05). The heart rate and mean arterial pressure in the observation group changed with time, and interactions between groups and time were both statistically significant(P<0.05).Conclusion Perioperative care based on Rogers’s science of unitary human beings can relieve the anxiety of children and improve their ability to regulate emotion in the face of difficulties and unfamiliar environments.
