摘 要:目的观察右美托咪定(dexmedetomidine,Dex)对脾切除术后老年大鼠蓝斑神经源性一氧化氮合酶(nNOS)、c-Fos和松驰素-3(relaxin-3)表达的影响,探讨Dex改善手术创伤应激的可能机制。方法清洁级18月龄SD雄性老年大鼠72只,体重500~600 g,随机分生理盐水组,脾切除模型组,Dex低剂量+脾切除组(3μg/kg)和Dex高剂量+脾切除组(12μg/kg),各组再分为1 d、3 d和7 d 3个亚组。采用免疫组织化学法检测蓝斑神经元中nNOS、c-Fos和relaxin-3免疫反应性,Western blot法检测蓝斑中nNOS和c-Fos水平。结果与生理盐水组比较,术后1 d和3 d模型组大鼠蓝斑nNOS、c-Fos、relaxin-3水平显著增高;与模型组比较,Dex低、高剂量组均能抑制脾切除所致蓝斑nNOS、c-Fos和relaxin-3水平升高。结论Dex能抑制老年大鼠脾切除术后蓝斑nNOS、c-Fos和relaxin-3表达,这可能是Dex改善手术创伤应激的机制之一。Objective To observe the effect of dexmedetomidine(Dex)on the expression of nNOS,c-Fos and relaxin-3 in locus coeruleus of aged rats after splenectomy,and to explore the possible mechanism of Dex alleviating surgical trauma stress.Methods 72 male Sprague-Dawley healthy rats at the age of 18 months,weighing 500-600 g,were randomly divided into normal saline group,splenectomy model group,Dex low dose(3μg/kg)+splenectomy group and Dex high dose(12μg/kg)+splenectomy group,each group was divided into three subgroups according to postoperative day 1,3 and 7.The immunoreactivities of nNOS,c-Fos and relaxin-3 in locus coeruleus neurons were detected by immunohistochemistry,and the levels of nNOS and c-Fos in the locus coeruleus were detected by Western blot.Results Compared with normal saline group,the immunoreactivities of nNOS,c-Fos and relaxin-3 in the splenectomy model group were remarkably increased in the locus coeruleus on postoperative day 1 and day 3.Compared with splenectomy model group,the levels of nNOS,c-Fos and relaxin-3 in both Dex high-dose group and Dex low-dose group were significantly decreased in the locus coeruleus.Conclusion Dex possibly inhibits the expression of nNOS,c-Fos and relaxin-3 in the locus coeruleus of aged rats after splenectomy,which may be one of mechanisms underlying the alleviatory effect of Dex on surgical trauma stress.
