温胃汤治疗慢性胃溃疡伴萎缩性胃炎痰瘀互结证型的疗效

摘　　要：目的:分析温胃汤治疗慢性胃溃疡伴萎缩性胃炎痰瘀互结证型的疗效。方法:选取昆山市中医医院2015年1月-2020年1月收诊的慢性胃溃疡伴萎缩性胃炎痰瘀互结证型患者60例,随机数字分组法分成对照组与观察组,每组30例。对照组患者口服胃复春片,观察组患者进行温胃汤治疗。比较两组患者临床、幽门螺杆菌(Helicobacter Pylori,Hp)感染根除情况、中医证候、血清炎性因子水平和不良反应。结果:观察组患者总有效率高于对照组(P<0.05)。观察组患者治疗后1个月、2个月和3个月Hp转阴率高于对照组,其中治疗后1个月、2个月Hp转阴率与对照组比较差异有统计学意义(P<0.05)。治疗后两组患者的胃痛、痞满、嘈杂、嗳气、纳呆、疲乏和口干评分较治疗前降低(P<0.05),观察组各项评分低于对照组(P<0.05)。观察组患者超敏C反应蛋白(High Sensitivity C-reactive Protein,hs-CRP)、白细胞介素-6(Interleukin 6,IL-6)和白细胞介素-8(Interleukin 8,IL-8)水平低于对照组(P<0.05)。观察组患者不良反应率高于对照组(P>0.05)。结论:温胃汤治疗慢性胃溃疡伴萎缩性胃炎痰瘀互结证型疗效和安全性较好,既能提升治疗效果,又能促使Hp根除,改善中医证候,建议使用。Objective:To investigate efficacy of the Wenwei decoction(温胃汤)on chronic gastric ulcer with atrophic gastritis of phlegm and blood stasis syndrome.Methods:60 cases from January 2015 to January 2020 were divided into two groups,each with 30 cases.The control group received Weifuchun tablets(胃复春片),and observation group received the Wenwei decoction.Results:The total efficiency in the observation group was higher than the control group(P<0.05).The Hp negative rate in observation group was higher than the control group at 1,2 and 3 months after treatment(P<0.05).After treatment,the scores of symptoms including stomachache,peepfullness,noise,belching,numbness,fatigue and dry mouth were decreased in both groups(P>0.05),and the observation group was lower(P<0.05).The levels of serum of hs-CRP,IL-6 and IL-8 in the observation group were lower than the control group(P<0.05).The adverse reaction rate in observation group was higher than the control group,with no significant difference(P>0.05).Conclusion:The Wenwei decoction on chronic gastric ulcer with atrophic gastritis of phlegm and blood stasis syndrome can effectively and safely promote the Hp negative rate,and ameliorate the TCM symptoms,which is worthy of recommendation.
