肩关节运动疗法用于甲状腺癌根治术后上肢功能康复的价值

摘　　要：目的探讨肩关节运动疗法用于甲状腺癌根治术后上肢功能康复的价值。方法选取首都医科大学附属北京同仁医院2019年1月~2021年1月收治的的甲状腺癌患者120例,以抽签随机法分为观察组与对照组各60例,两组均行甲状腺癌根治术,术后对照组采取常规康复锻炼,观察组采取肩关节运动疗法锻炼,比较两组术后1周、3个月、6个月的肩功能评分、颈部运动度。结果两组术后不同时点的肌力、日常生活能力、肩关节活动范围评分呈现上升趋势,疼痛程度评分呈现下降趋势,观察组术后1周日常生活能力评分及术后1周、3个月的肩关节活动范围评分显著高于对照组,差异有统计学意义(P<0.05);两组术后不同时点颈部运动度各指标评分呈上升趋势,观察组术后1周颈部运动度各指标评分均高于对照组,且术后3个月向健侧屈、向患侧屈、向健侧旋转评分显著高于对照组,差异有统计学意义(P<0.05);两组患者术后3个月的免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)水平均明显高于术后1周,观察组患者术后3个月的IgA、IgG、IgM水平明显高于对照组,差异有统计学意义(P<0.05);两组术后3个月的疼痛康复情况比较差异无统计学意义(P>0.05)。结论使用肩关节运动疗法进行术后康复训练对甲状腺癌患者的肩颈功能和免疫功能等指标都有明显作用。Objective To explore the value of shoulder joint exercise therapy in the rehabilitation of upper limb function after radical thyroidectomy.Methods 120 cases of thyroid cancer patients admitted to our hospital from January 2019 to January 2021 were selected as the research objects,and randomly divided into the observation group and the control group,60 cases in each group.Both groups underwent radical thyroid cancer surgery.The control group took routine rehabilitation exercise after surgery,and the observation group took shoulder exercise therapy,The two groups were compared in terms of shoulder function score and neck motion at 1 week,3 months and 6 months after surgery,immune function indexes 1d and 3 months after surgery,and pain recovery at 3 months after surgery.Results Two groups of different time points after muscle,daily life ability,shoulder joint range score rise,the degree of pain scores showed a trend of decline,the difference was statistically significant(P<0.05),the observation group after 1 week daily life ability score and postoperative 1 week and 3 months of shoulder joint range score is significantly higher than the control group,The difference was statistically significant(P<0.05);Different points in the two groups of postoperative neck movement degree of each index score is on the rise,the difference was statistically significant(P<0.05),the observation group in postoperative 1 week and neck motion degree of each index score were higher than the control group,and postoperative 3 months to health lateral flexion,to develop lateral flexion,spinning in the contralateral score is significantly higher than the control group,the difference was statistically significant(P<0.05);The levels of immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)in 2 groups 3 months after surgery were significantly higher than those in 1 week after surgery,while the levels of IgA,IgG and IgM in observation group were significantly higher than those in control group 3 months after surgery,the difference w
