摘  要:目的探讨动脉粥样硬化型脑梗死患者血清巨噬细胞游走抑制因子(MIF)水平与颅内动脉粥样硬化(ICAS)及其狭窄程度的相关性。方法选取2019年9月至2020年1月在新乡医学院第一附属医院神经内科就诊的脑梗死患者260例,所有研究对象均有完整的磁共振血管成像(MRA)结果,根据其MRA所示颅内动脉粥样硬化程度将研究对象分为无ICAS组61例、有ICAS组199例(轻度88例、中度49例、重度62例)。分析各组间血清MIF的水平差异,进一步用Spearman相关性的方法分析血清MIF与ICAS及其狭窄程度的关系。结果与无ICAS组相比,有ICAS组中的血同型半胱氨酸(Hcy)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FPG)、MIF水平明显升高,而高密度脂蛋白胆固醇(HDL-C)水平明显降低,差异具有统计学意义(F=-11.680、-2.111、-4.740、-19.482、3.975,P<0.05);多因素Logistic分析结果表明高Hcy、LDL-C、FPG、MIF及低HDL-C是颅内动脉粥样硬化的独立危险因素。相关性结果表明血清MIF水平与颅内动脉粥样硬化狭窄程度呈正相关性(r=0.657,P<0.01),血清MIF诊断ICAS的ROC曲线下面积为0.897,敏感度为0.775,特异度为0.846。结论MIF是动脉粥样硬化性脑梗死患者颅内动脉粥样硬化的独立危险因素,且与颅内动脉粥样硬化狭窄程度相关。Objective To investigate the correlation between the level of serum macrophage migration inhibitory factor(MIF)and the degree of intracranial atherosclerotic stenosis(ICAS)in patients with atherosclerotic cerebral infarction.Methods Patients with cerebral infarction from September 2019 to January 2020 in the department of neurology of the First Affiliated Hospital of Xinxiang Medical College were selected,who had complete results of magnetic resonance angiography(MRA).The subjects were divided into non-ICAS group of 61 cases and ICAS group of 199 cases(88 cases of mild,49 cases of moderate,62 cases of severe)according to the degree of internal atherosclerosis.The differences in the level of serum MIF between the groups were analyzed,and the relationship between the level of serum MIF and ICAS and its the degree was further analyzed by the method of Spearman correlation.Results Compared with the non-ICAS group,the level of the blood homocysteine(Hcy),low-density lipoprotein(LDL-C),fasting blood glucose(FPG),and MIF levels in the ICAS group were significantly higher,while the level of the high-density lipoprotein(HDL-C)was significantly reduced,and the differences were statistically significant(t=-11.680,-2.111,-4.740,-19.482,3.975;P<0.05).Multivariate Logistic analysis showed that high Hcy,LDL-C,FPG,MIF and low HDL-C were the independent risk factors for intracranial atherosclerosis.The correlation result showed that the level of the serum MIF was positively correlated(r=0.657,P<0.01).The area under the ROC curve for the diagnosis of ICAS by serum MIF was 0.897,sensitivity was 0.775,and specificity was 0.846.Conclusion MIF is an independent risk factor for intracranial atherosclerosis in patients with atherosclerotic cerebral infarction,and it is related to the degree of intracranial atherosclerotic stenosis.
