颅脑外伤性癫痫发病的危险因素分析

摘　　要：目的分析颅脑外伤性癫痫发病的危险因素。方法回顾性选取2016年6月—2018年6月汕头市第二人民医院收治441例颅脑外伤患者的临床资料,根据是否继发癫痫分为癫痫组和对照组,癫痫组55例,对照组386例。统计2组患者的临床资料,进行单因素分析和多因素Logistic回归分析。结果单因素分析显示,癫痫组患者酗酒率、合并硬膜破裂、合并皮质挫裂伤、凹陷性颅骨骨折、颅内血肿、合并症占比高于对照组,外伤程度比例低于对照组(P<0.05);多因素Logistic回归结果显示,硬膜破裂、外伤程度、皮质挫裂伤、凹陷性颅骨骨折、颅内血肿是颅脑外伤性癫痫发病的危险因素。结论硬膜破裂、外伤程度、皮质挫裂伤、凹陷性颅骨骨折、颅内血肿患者并发颅脑外伤性癫痫的概率明显增加,上述类型患者需予以重点关注。Objective To explore the risk factors of post-traumatic epilepsy.Methods The clinical data of patients with craniocerebral trauma admitted from June 2016 to June 2018 were reviewed and divided into epilepsy group(n= 55) and control group(n= 386) according to the occurrence of secondary epilepsy.The clinical data of the two groups were collected by univariate analysis and multivariate Logistic regression analysis.Results Univariate analysis showed that the proportion of patients with alcohol abuse,dural rupture,cortical contusion,depressed skull fracture,intracranial hematoma,and comorbidities in the epilepsy group were higher than those in the control group,degree of trauma was better than that of the control group(P< 0.05);multivariate Logistic regression analysis showed that the dural rupture,degree of trauma,cortical contusion and laceration,depressed skull fracture and intracranial hematoma were the risk factors of post-traumatic epilepsy(P< 0.05).Conclusion The probability of post-traumatic epilepsy is significantly increased in patients with dural rupture,degree of trauma,cortical contusion and laceration,sunken skull fracture and intracranial hematoma,which should be paid more attention to.
