摘 要:目的探讨“中医+”思维的住院医师规范化培学员的中医临床思维培养的作用。方法2017年9月—2018年9月抽取湖南中医药大学第二附属医院2017级硕士研究生60名为研究对象进行培训,采用随机数表法,将其随机均分为两组,分别为观察组为“中医+”组(30名),对照组为“传统规培组”(30名),培训满1年后比较自我评价、各项学习任务完成情况和学员年度综合成绩。结果观察组进行住院医师规范化培训对学员的中医兴趣培养、临床诊断能力的提升、自我学习能力的提高、自我时间规划能力的把控、中医临床思维能力的提高及临床知识的全面掌控的效果优于对照组,差异有统计学意义(P<0.05)。观察组和对照组的同学最终都能有效完成本学年安排的规范化培训学习任务,差异无统计学意义(P>0.05)。观察组理论考核成绩为(90.93±3.92)分,临床操作考核成绩为(90.13±2.58)分,成绩均优于对照组,差异有统计学意义(P<0.05)。结论采用“中医+”思维在能够有效完成任务的情况下,更能提高学员对中医学习的兴趣、中医临床思维能力的培养、知识的掌握程度以及分析解决问题的能力,在提高中医基础理论的同时,能强化中医临床思维与临床经验,提高住院规培医师的动手能力,同时也通过思维培养提高医师对中医疾病的认识与了解,值得在临床中推广与运用。Objective To explore the role of TCM clinical thinking training for residents in standardized training of"TCM+"thinking.Methods From September 2017 to September 2018,a total of 60 postgraduates in the Second Affiliated Hospital of Hunan University of Traditional Chinese Medicine in 2017 were selected as the research subjects for training.Using the random number table method,they were randomly divided into two groups.The observation group was the"TCM+"group(30 people),and the control group was the"traditional training group"(30 people).After one year of training,compared the self-evaluation,the completion of various learning tasks,and the students'annual comprehensive scores.Results The standardized training of residents in the observation group had better effects on the cultivation of students'interest in TCM,the improvement of clinical diagnosis ability,the improvement of self-learning ability,the control of self-time planning ability,the improvement of TCM clinical thinking ability and the overall control of clinical knowledge compared with the control group,the difference was statistically significant(P<0.05).The students in the observation group and the control group were able to effectively complete the standardized training and learning tasks arranged in this school year,and the difference was not statistically significant(P>0.05).The theoretical test score of the observation group was(90.93±3.92)points,and the clinical operation test score was(90.13±2.58)points,both of which were better than those of the control group,and the difference was statistically significant(P<0.05).Conclusion"TCM+"thinking can improve the students'interest in TCM learning,the cultivation of TCM clinical thinking ability,the mastery of knowledge and the ability to analyze and solve problems when they can effectively complete the task.While improving the basic theory of traditional Chinese medicine,strengthen the clinical thinking and clinical experience of traditional Chinese medicine,and improve the practical ability of inpatien
