摘　　要：目的探讨急性缺血性卒中患者空腹血糖水平的季节性变化。方法回顾性纳入2016年1月1日至2020年12月31日期间在天津医科大学第二医院神经内科住院治疗且具备空腹血糖数据的急性缺血性卒中患者。收集所有患者一般资料和空腹血糖结果,分析各项指标的季节性变化。采用多元线性回归分析确定空腹血糖的影响因素。结果共纳入1323例具备空腹血糖化验数据的急性缺血性卒中患者,其中519例(39.2%)合并糖尿病,804例(60.8%)不合并糖尿病。冬季组患者基线美国国立卫生研究院卒中量表评分、体重指数、收缩压、舒张压和空腹血糖水平均显著高于夏季组(P均<0.05)。进一步对性别和年龄进行的亚组分析显示,65~80岁老年患者空腹血糖水平具有显著季节性变化(P<0.05)。多元线性回归分析显示,年龄、季节、体重指数、是否合并糖尿病、是否使用胰岛素和(或)口服降糖药是空腹血糖水平的独立影响因素。结论急性缺血性卒中患者空腹血糖水平具有季节性波动特点,这种现象在65~80岁老年患者中更为明显。Objective To investigate the seasonal changes of fasting plasma glucose(FPG)levels in patients with acute ischemic stroke.Methods Patients with acute ischemic stroke who were hospitalized in the Department of Neurology,the Second Hospital of Tianjin Medical University and had fasting blood glucose data from January 1,2016 to December 31,2020 were enrolled retrospectively.The general data and FPG results of all patients were collected,and the seasonal changes of various indexes were analyzed.Multiple linear regression analysis was used to determine the influencing factors of FPG.Results A total of 1323 patients with acute ischemic stroke who had FPG data were enrolled,of whom 519 patients(39.2%)had diabetes mellitus and 804(60.8%)had no diabetes mellitus.The baseline National Institutes of Health Stroke Scale score,body mass index,systolic blood pressure,diastolic blood pressure and FPG in the winter group were significantly higher than those in the summer group(all P<0.05).Further subgroup analysis of gender and age showed that the FPG level of elderly patients aged 65-80 years had significant seasonal changes(P<0.05).Multiple linear regression analysis showed that age,season,body mass index,whether complicated with diabetes mellitus,whether using insulin and/or oral hypoglycemic drugs were the independent influencing factors of FPG levels.Conclusion The FPG levels in patients with acute ischemic stroke have the characteristics of seasonal fluctuation,which is more obvious in elderly patients aged 65-80 years.
