摘  要:目的观察吸烟对青少年心电图参数T波峰末间期(Tp-e间期)、Tp-e/QT和Tp-e/QTc比值的影响,探讨吸烟与室性心律失常发生风险的相关性。方法选择305例青少年为研究对象,将烟龄≥1年,每日吸烟支数≥3支受试者归为吸烟组(152例),其他归为对照组(153例)。所有受试者均行常规体表12导联心电图检查获得Tp-e间期、Tp-e/QT和Tp-e/QTc比值数据,比较组间差异。分析每日吸烟量、烟龄、开始吸烟年龄与Tp-e间期、Tp-e/QT和Tp-e/QTc比值相关性。结果吸烟组Tp-e间期、Tp-e/QT比值、Tp-e/QTc比值均高于对照组[(96.1±10.8)ms vs(74.7±9.6)ms,0.25±0.03 vs 0.20±0.03,0.23±0.03 vs 0.18±0.03,P均<0.05]。烟龄6年组Tp-e间期、Tp-e/QT比值、Tp-e/QTc比值高于1~3年组、4~5年组(P均<0.05),每日吸烟量>10支组QT间期、Tp-e间期、Tp-e/QT比值、Tp-e/QTc比值高于3~5支组、5~10支组(P均<0.05),开始吸烟年龄12~13岁组Tp-e间期、Tp-e/QT比值、Tp-e/QTc比值高于13~14岁组、15岁组(P均<0.05)。Spearman相关分析结果示Tp-e/QT比值、Tp-e/QTc比值与开始吸烟年龄呈负相关(rs=-0.251、-0.213,P<0.05)。多重线性回归分析结果示开始吸烟年龄与Tp-e/QT比值、Tp-e/QTc比值关系均呈线性关系(P<0.05)。结论吸烟可导致青少年心电图Tp-e间期、Tp-e/QT、Tp-e/QTc比值增高,开始吸烟年龄越早对心室复极影响越大。Objective To observe the effects of smoking on the Tp-e interval,Tp-e/QT and Tp-e/QTc ratio of electrocardiogram parameters in adolescents,and to explore the correlation between smoking and the risk of ventricular arrhythmia.Methods Three hundred and five adolescents were selected as the study subjects.Those who smoked for more than 1 year and smoked more than 3 cigarettes per day were divided into smoking group(152 cases)and control group(153 cases).Tp-e interval,Tp-e/QT and Tp-e/QTc ratio were obtained by routine 12-lead electrocardiogram(ECG)in all subjects,and the differences between groups were compared.The correlation of daily smoking volume,smoking age and starting smoking age with Tp-e interval,Tp-e/QT and Tp-e/QTc ratio was analyzed.Results Tp-e interval,Tp-e/QT ratio and Tp-e/QTc ratio in smoking group was higher than that of the control group[(96.1±10.8)ms vs(74.7±9.6)ms,0.25±0.03 vs 0.20±0.03,0.23±0.03 vs 0.18±0.03,all P<0.05].Tp-e interval,Tp-e/QT ratio,Tp-e/QTc ratio in 6-year smoking age group were higher than those in 1-3-year group and 4-5-year group(all P<0.05).The QT interval,Tp-e interval,Tp-e/QT ratio,Tp-e/QTc ratio in daily smoking amount>10 Branches were higher than that in 3-5-branch group and 5-10 branch group(all P<0.05).The Tp-e interval,Tp-e/QT ratio,Tp-e/QTc ratio in group of 12-13 years old who started smoking were higher than that of 13-14 years old group and 15 years old group(all P<0.05).Spearman correlation analysis showed that Tp-e/QT ratio and Tp-e/QTc ratio were negatively correlated with the age at which smoking began(rs=0.251,-0.213,P<0.05).Multiple linear regression analysis showed that there was a linear relationship between smoking age and Tp-e/QTc ratio and Tp-e/QTc ratio(P<0.05).Conclusion Smoking can increase the Tp-e interval,Tp-e/QT,Tp-e/QTc ratio of ECG in adolescents.The earlier the smoking age begins,the greater the effect on ventricular repolarization.[Chinese Journal of Cardiac Pacing and Electrophysiology,2022,36(1):47-51]
