摘　　要：目的探讨经腹膜外结肠造口对腹腔镜辅助经腹会阴联合直肠癌根治术(APR)患者造口旁疝发生率的影响。方法回顾性分析2017年2月至2020年6月福建省立医院胃肠外科收治的74例行腹腔镜辅助APR治疗的低位直肠癌患者的临床资料,根据造口方式不同分为试验组(经腹膜外结肠造口,44例)和对照组(经腹膜结肠造口,30例),比较两组的造口旁疝、造口皮肤黏膜分离、造口坏死及造口感染等并发症的发生率。结果两组造口皮肤黏膜分离、造口坏死及造口感染发生率比较,差异无统计学意义(P>0.05);试验组造口旁疝发生率低于对照组,差异有统计学意义(P<0.05)。结论腹腔镜辅助APR治疗直肠癌患者时采用经腹膜外结肠造口可明显降低造口旁疝的发生率。Objective The effect of end-sigmoid colostomy through the extraperitoneal route on the incidence of parastomal hernia after laparoscopy-assisted abdominoperineal resection(APR)was explored.Methods The clinical data of a total of 74 patients who were diagnosed with low rectal cancer and underwent laparoscopy-assisted APR in the Department of Gastrointestinal Surgery at Fujian Provincial Hospital from February 2017 to June 2020 were retrospectively analyzed.They were divided into a test group(extraperitoneal route,44 cases)and a control group(transperitoneal route,30 cases)according to the colostomy mode.The incidences of complications such as parastomal hernia,stoma cutaneous mucosa separation,stoma necrosis and stoma infection were compared in the two groups.Results When the incidences of stoma cutaneous mucosa separation,stoma necrosis and stoma infection were compared in the two groups,the differences were not statistically significant(P>0.05);The incidence of parastomal hernia in the test group was lower than that in the control group,with statistically significant difference(P<0.05).Conclusion The end-sigmoid colostomy through the extraperitoneal route after laparoscopy-assisted APR for treating patients with rectal cancer can significantly reduce the incidence of parastomal hernia.
