宫颈上皮内瘤变患者行LEEP刀宫颈环切术深度对妊娠率及妊娠结局的影响

摘　　要：目的探讨子宫颈上皮内瘤变患者行LEEP刀宫颈环切术深度对妊娠率及妊娠结局的影响。方法便利选取该院2016年3月—2017年3月确诊的子宫颈上皮内瘤变CINⅡ~Ⅲ级并拟行宫颈环切术的未孕者80例为研究对象,根据切割深度(≥或<15 mm)分为观察组和对照组。观察组切割深度≥15 mm,对照组<15 mm。术后对两组患者进行随访,对比分析两组患者受孕时间及妊娠结局有无差异。结果术后随访3年,观察组成功妊娠28例,妊娠率为70.0%,对照组成功妊娠31例,妊娠率为77.5%,两组比较差异无统计学意义(χ^(2)=0.581,P>0.05)。两组孕妇妊娠结局方面,胎儿病死率、剖腹产率、妊娠时间、新生儿体质量及Apgar评分差异无统计学意义(P>0.05);术后随访3年,对照组复发6例,观察组复发0例,对照组术后复发风险高于观察组,差异有统计学意义(χ^(2)=4.505,P<0.05)。结论宫颈上皮内瘤变患者行LEEP刀宫颈环切术深度对妊娠率及妊娠结局无明显影响,但切割深度≥15 mm可显著降低术后复发风险。Objective To investigate the influence of the depth of LEEP knife circumcision on pregnancy rate and pregnancy outcome in patients with cervical intraepithelial neoplasia.Methods A total of 80 non-pregnant patients with cervical intraepithelial neoplasia CINⅡ-Ⅲwho were diagnosed with cervical intraepithelial neoplasia from March 2016 to March 2017 and planned to undergo cervical circumcision were conveniently selected as the research objects.According to the cutting depth(greater than or less than 15mm)were divided into observation group and control group.The cutting depth of the observation group was≥15 mm,and the control group was<15 mm.After the operation,the two groups of patients were followed up,and the two groups of patients were compared and analyzed for differences in pregnancy time and pregnancy outcome.Results After 3 years of follow-up,28 cases were successfully pregnant in the observation group,the pregnancy rate was 70.0%,and 31 cases in the control group were successfully pregnant,and the pregnancy rate was 77.5%.There was no statistically significant difference between the two groups(χ^(2)=0.581,P>0.05).In terms of pregnancy outcomes between the two groups,there was no statistically significant difference in fetal mortality,caesarean section rate,pregnancy time,newborn weight and Apgar score(P>0.05).After 3 years of follow-up,there were 6 recurrences in the control group and 0 recurrences in the observation group,the risk of recurrence in the control group was higher than that in the observation group,and the difference was statistically significant(χ^(2)=4.505,P<0.05).Conclusion In patients with cervical intraepithelial neoplasia,the depth of circumcision with LEEP knife has no significant effect on pregnancy rate and pregnancy outcome,but the cutting depth≥15mm can significantly reduce the risk of postoperative recurrence.
