摘　　要：目的:评价内镜黏膜下剥离术(ESD)治疗表浅食管鳞癌及癌前病变的长期临床疗效和安全性。方法:回顾性分析经ESD治疗的66例表浅食管鳞癌及癌前病变患者,评价ESD治疗表浅食管鳞癌及癌前病变的临床疗效。结果:66例患者共切除73处病变,术中发生穿孔1例,贲门黏膜撕裂1例,出血5例,整块切除率、完全切除率、治愈性切除率分别为90.4%、89.0%、79.5%。术后随访中位数76个月,发生术后感染2例,食管狭窄3例,复发1例,新发1例,随访5年整体生存率96.5%,肿瘤特异性生存率97.0%。结论:ESD治疗表浅食管鳞癌和癌前病变是安全有效的。Objective To evaluate the long-term clinical efficacy and safety of endoscopic submucosal dissection(ESD)in the treatment of superficial esophageal squamous cell carcinoma and precancerous lesions.Method 66 patients with superficial esophageal squamous cell carcinoma and precancerous lesions treated by ESD in our hospital were analyzed retrospectively to evaluate the clinical efficacy of ESD in the treatment of superficial esophageal squamous cell carcinoma and precancerous lesions.Results A total of 73 lesions were removed in 66 patients,perforation occurred in 1 case,cardiac orifice mucosal laceration in 1 case and hemorrhage in 5 cases.The overall resection rate,complete resection rate and curative resection rate were 90.4%,89.0%and 79.5%,respectively.The median follow-up was 76 months,with 2 cases of postoperative infection,3 cases of esophageal stricture,1 case recurred and 1 new case occurred during this period.Follow up for 5 years showed that the overall survival rate was 96.5%and the tumor specific survival rate was 97.0%.Conclusion ESD is a safe and effective approach in treatment of superficial esophageal squamous cell carcinoma and precancerous lesions.
