摘　　要：目的探讨鼻内镜下暴露泪总管治疗小泪囊慢性泪囊炎的手术效果,为提高小泪囊慢性泪囊炎手术的远期疗效提供有效的改进方案。方法回顾性病例对照研究。选取本院2016年1月至2018年12月期间所收治的94例(106只眼)小泪囊慢性泪囊炎患者作为研究对象,均施行鼻内镜下泪囊鼻腔吻合术。随机分为2组,A组44例(52只眼)施行鼻内镜下泪囊鼻腔吻合术,术中暴露至泪总管;B组患者50例(54只眼)施行常规鼻内镜下泪囊鼻腔吻合术。术后1、3、6、12个月随访4次,检查吻合口开放情况。结果远期随访A组治愈率(98.08%)高于B组(85.19%),差异有统计学意义(P<0.05)。结论采用鼻内镜下泪总管暴露治疗小泪囊慢性泪囊炎远期成功率高,是治疗小泪囊慢性泪囊炎的有效方法。Objective To investigate the surgical effect of exposing the common lacrimal duct under nasal endoscope in the treatment of chronic dacryocystitis of the small lacrimal sac,to provide an effective improvement scheme for improving the long-term effect of the operation.Methods Retrospective case-comtrol study.A total of 94 cases(106 eyes)with chronic dacryocystitis of the small lacrimal sac treated in our hospital from January 2016 to December 2018 were included in the analysis.The patients were randomly divided into two groups.Group A:44 cases(50 eyes)were treated by dacryocystorhinostomy under nasal endoscope,during which the common lacrimal duct was exposed.Group B:50 cases(54 eyes)were treated by the traditional endoscopic dacryocystorhinostomy.All patients were followed up four times at 1,3,6 and 12 months after the surgery,respectively.As well as underwent nasal endoscopy and lacrimal passage irrigation.Results At twelve months after the surgery,the cure rate in group A(98.08%)was higher than that in group B(85.19%),the difference was statistically significant(P<0.05).Conclusions Treatment of chronic dacryocystitis of the small lacrimal sac by exposing the lacrimal duct under nasal endoscopy has a high long-term success rate.It is an effective method for the treatment of chronic dacryocystitis of the small lacrimal sac.
