前列地尔联合长春西汀对后循环缺血性眩晕患者DHI评分、血流流变学及血管内皮功能的影响

摘　　要：目的探讨前列地尔联合长春西汀治疗后循环缺血性眩晕患者的临床效果。方法选取2020年1月至2021年1月我院神经内科收治的86例后循环缺血性眩晕患者作为研究对象,随机将其分为对照组与观察组,各43例。对照组给予长春西汀治疗,观察组给予前列地尔联合长春西汀治疗。比较两组患者的临床效果。结果观察组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的DHI各项评分均降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的血浆黏度、红细胞聚集指数、纤维蛋白原水平均降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的BA、RVA及LVA平均血流速度均升高,且观察组高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的NO、ET-1水平均改善,且观察组优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的ADL、SF-36评分均升高,且观察组高于对照组,差异具有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论前列地尔联合长春西汀治疗后循环缺血性眩晕患者的临床效果显著,可改善患者的眩晕症状、血流流变学指标及椎基底动脉血流速度,提高患者的血管内皮功能、日常生活能力及生活质量,值得临床推广应用。Objective To investigate the clinical effect of alprostadil combined with vinpocetine in the treatment of patients with posterior circulation ischemic vertigo.Methods A total of 86 patients with posterior circulation ischemic vertigo treated in theneurology department of our hospital from January 2020 to January 2021 were selected as the research objects.The patients were randomly divided into control group and observation group,with 43 cases in each group.The control group was treated with vinpocetine,and the observation group was treated with alprostadil combined with vinpocetine.The clinical effect of the two groups was compared.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of DHI in the two groups decreased,and those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).After treatment,the plasma viscosity,erythrocyte aggregation index and fibrinogen level in the two groups decreased,and those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).After treatment,the average blood flow velocities of BA,RVA and LVA in the two groups increased,and those in the observation group were higher than the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of NO and ET-1 in the two groups improved,and those in the observation group were better than the control group,and the differences were statistically significant(P<0.05).After treatment,the scores of ADL and SF-36 in the two groups increased,and those in the observation group were higher than the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Alprostadil combined with vinpocetine in the treatment of
