疏肝散结法对甲状腺癌术后抑郁及相关细胞因子的影响

摘　　要：目的:观察疏肝散结法对甲状腺癌术后患者抑郁状态及相关细胞因子的影响。方法:随机将60例甲状腺癌术后患者分为对照组和观察组,每组患者各30例,对照组给予疏肝解郁胶囊口服,观察组接受疏肝散结法治疗,治疗2个月。治疗前后进行抑郁评分,并ELISA检测血清5-羟色胺(5-HT)、多巴胺(DA)、胰岛素样生长因子-1(IGF-1)、血管内皮生长因子(VEGF)水平;应用生存质量(QOD)量表进行生活质量评价。结果:治疗2个月后,2组患者HAMD及SDS抑郁评分均明显下降(P<0.05),血清5-HT、DA水平明显升高(P<0.05),IGF-1、VEGF水平明显降低(P<0.05);观察组上述变化均较对照组显著(P<0.05)。2组生活质量评分治疗后均有所提高(P<0.05),观察组提高更明显(P<0.05)。相关分析显示:患者血清5-HT、DA水平与IGF-1呈负相关,VEGF水平与IGF-1呈正相关(P<0.05)。结论:疏肝散结法可有效减轻甲状腺癌术后患者的抑郁症状,提高血清5-HT和DA水平,降低血清IGF-1、VEGF水平,提升生活质量,抑制肿瘤复发。Objective:To investigate the efficacy of the method of dispersing stagnation of liver-qi on the depressed state and the related cytokines of patients with thyroid cancer after operation.Methods:60 cases of thyroid cancer patients after operation were divided into two group randomly,30 cases in each group.The control group were treated with Shugan Jieyu Capsule orally.the observation group were treated with method of dispersing stagnation of liver-qi.The levels of serum 5-HT,DA,IGF-1,VEGF were detected by ELISA method and the depression score and quality of life score were recorded.Results:After 2 months of treatment,the HAMD and SDS scors were lowered inthe two groups,the levels of serum 5-HT,DA were raised(P<0.05),the IGF-1 and VEGF levels were lowered(P<0.05);all above changes in observation group were more sigbificant than those in control group(P<0.05).The correlation analysis showe d that 5-HT and DA levels were negatively correlated with IGF-1,VEGF levels were positively correlated with IGF-1(P<0.05).Conclusion:The method of dispersing stagnation of liver-qi can significantly improve the levels of serum 5-HT,DA of patients with thyroid cancer after operation,reduce IGF-1,VEGF levels and this method could improve the patients’depression and quality of their life,suppress cancer recurrence.
