两种正畸矫治器联合微种植体支抗对先天性缺牙患者美观及咬合功能恢复的比较

摘　　要：目的:探讨无托槽隐形矫治联合微种植体支抗对先天性缺牙美观及咬合功能的影响。方法:选择临夏市人民医院2016年5-2018年5月收治的先天性缺牙患者105例,随机分为两组:Nc组(自锁陶瓷托槽联合微种植体支抗)51例,Yk组(无托槽隐形矫治联合微种植体支抗)54例,观察两组的修复效果、咀嚼效能及牙周状况,整齐美观度及舒适度改变情况,对比两组菌斑指数(PLI)、牙龈指数(GI)和牙釉质脱矿情况。结果:牙齿修复效果比较,Yk组优良率96.1%,Nc组85.1%,两组比较差异有统计学意义(P<0.05);Yk组、Nc组矫治前咀嚼效能比较无统计学意义(P>0.05),与Yk组矫治后比较,Nc组矫治后1个月、6个月后明显降低,两组比较差异显著,具有统计学意义(P<0.05);与Yk组整齐美观度、舒适度、固位能力比较,Nc组明显下降,两组比较差异显著,具有统计学意义(P<0.05)。与Yk组PLI、GI、牙釉质脱矿情况比较,Nc组明显下降,差异显著,具有统计学意义(P<0.05)。两组牙周状况比较,Yk组优于Nc组,差异具有统计学意义(P<0.05)。结论:无托槽隐形矫治联合微种植体支抗治疗先天性缺牙效果较好,美观度较高,可提高患者咬合功能。Objective To investigate the effect of invisible correction without bracket combined with micro implant anchorage on the aesthetics and occlusal function of congenital missing teeth.Methods 105 patients with congenital missing teeth treated in Linxia people’s Hospital from May 2016 to may 2018 were selected,They were randomly divided into two groups:51 cases in Nc group (self-locking ceramic bracket combined with Micro Implant Anchorage) and 54 cases in Yk group (invisible correction without bracket combined with Micro Implant Anchorage).The repair effect,masticatory efficiency,periodontal condition,neat appearance and comfort changes of the two groups were observed,and the plaque index (PLI),gingival index(GI) and enamel demineralization of the two groups were compared.Results In Yk group,there were 19 patients with excellent results,30 patients with good results,only 2 patients with poor results,only 11 patients with excellent results in Nc group,35 patients with good results,8 patients with poor results,6 more than in Yk group.The excellent and good rate was 96.1% in Yk group and 85.1% in Nc group (P<0.05).There was no significant difference in masticatory efficacy between Yk group and Nc group (P>0.05).Compared with Yk group,Nc group showed a significant decrease in neat appearance,comfort and retention ability (P<0.05).Compared with the PLI,GI and enamel demineralization of Yk group,the demineralization of Nc group decreased significantly,and the difference between Yk group and Nc group was significant (P<0.05).There were 8 more normal periodontal patients in Yk group than in Nc group,3 mild patients,1 severe patient,13 mild lesions in Nc group,2 moderate binsun patients,and 2 severe lesions in Nc group.The difference between Yk group and Nc group was significant (P<0.05).Conclusion The effect of invisible orthodontic treatment without bracket combined with oral implant repair in the treatment of congenital missing teeth is good,the aesthetic degree is high,and the occlusal function of patients can be impr
