益心通痹合剂对胸痹心痛病(劳力性心绞痛)合并冠脉慢血流患者ALDH2不同表型疗效的随机对照研究

摘　　要：目的:观察益气活血法对胸痹心痛病(劳力性心绞痛)合并冠脉慢血流在不同ALDH2表型患者的疗效。方法:选择诊断为胸痹心痛病(劳力性心绞痛)合并冠脉慢血流气虚血瘀型的患者120例,按ALDH2基因型将上述患者归为野生型(GG型)80例与非野生型(非GG型)40例,将GG型与非GG型纳入患者随机分为试验组与对照组,其中GG型试验组与对照组各40例,非GG型试验组与对照组各20例,所有试验组患者给予益心通痹合剂口服治疗,所有对照组患者给予单硝酸异山梨酯缓释片口服治疗,疗程均为4周。观察对比不同基因分型的试验组与对照组的有效率、心电图QT间期离散度、西雅图积分变化和不良反应情况。结果:在GG型患者中试验组和对照组的组间有效率和西雅图积分对比,差异没有统计学意义(P>0.05);试验组和对照组的西雅图积分和心电图QT离散度治疗前、后差异均有统计学意义(P<0.05)。在非GG型患者中试验组和对照组的有效率、西雅图积分组间对比,差异有统计学意义(P<0.05);试验组和对照组西雅图积分和心电图QT离散度治疗前、后的组内对比,差异均有统计学意义(P<0.05)。研究期间所有患者均无不良事件发生。结论:益心通痹合剂能改善ALDH2不同表型胸痹心痛病(心绞痛)合并冠脉慢血流患者的临床症状。Objective:To observe the therapeutic effect of Yixin Tongbi Mixture on chest stuffiness and pains(angina pectoris)complicated with coronary slow flow in different phenotype of ALDH2.Methods:120 patients with chest arthralgia(angina pectoris)complicated with coronary slow flow and Qi deficiency and Blood stasis were brought into this study.All of the patients were divided into wild type group(GG type)with 80 patients and non-wild type group(non-GG type)with 40 patients according to ALDH2 genotype.Patients in both groups were further divided into test group and control group randomly.Genotype of GG had 40 cases in each group and genotype of non-GG had 20 cases in each group.The course of treatment was 4 weeks.All patients in test group were prescribed with Yixin Tongbi Mixture and isosorbide mononitrate sustained release tablets in control group.The effective rate,the change of Seattle score and adverse reactions before and after treatment were observed and compared in different genotypes.Results:In GG patients there was no significant difference in the effective rate and Seattle score between the experimental group and the control group(P>0.05);in the same group,there were significant differences in Seattle score and QT dispersion before and after treatment(P<0.05).In non-GG patients the effective rate between the experimental group and the control group was significantly different(P<0.05),and the Seattle score between the groups after treatment was significantly different(P<0.05);when compared with the same group,the Seattle score and QT dispersion before and after treatment in the experimental group and the control group were significantly different(P<0.05).No adverse events occurred in all patients during the study period.Conclusion:Yixin Tongbi Mixture can improve the clinical symptoms in patient with chest stuffiness and pains(angina pectoris)complicated with coronary slow flow in different phenotypes of ALDH2 equally.
