摘 要:目的评价3D打印联合计算机导航技术在眼眶骨折修复术中应用的临床效果。方法回顾性病例系列研究。分析扬州大学附属医院2019年1月至2021年2月3D打印联合计算机导航技术辅助眼眶骨折修复手术39例的临床资料,其中爆裂性骨折29例,眶颧颌骨折10例。术后随访6~12个月观察术后并发症、解剖复位、复视及眼球运动障碍程度等。结果随访期间,未出现视神经损伤或视力丧失等严重并发症,术后CT检查骨折得到良好的解剖复位。术后6个月复视程度较术前明显改善(Z=-3.64,P<0.001);眼球运动障碍程度较术前明显改善(Z=-3.31,P=0.001)。双眼球凹陷差值由术前2.50(2.00,3.00)mm降低至术后1周的0.00(0.00,1.00)mm(Z=-6.09,P<0.001);双侧眼眶容积差值由术前(2.31±0.63)mm^(3)降低至术后1周(0.47±0.38)mm^(3)(t=16.82,P<0.001);10例眶颧颌骨折患者眶区对称度差值由术前(8.72±1.05)mm降低至术后1周的(1.20±0.49)mm(t=23.39,P<0.001)。结论3D打印联合计算机导航技术在眼眶骨折的整复手术中应用效果良好。Objective To evaluate the clinical efficacy of 3D printing combined with computer navigation technique in the treatment for orbital fractures.Methods This was a retrospective case series study.The clinical data of 39 cases with 3D printing combined with computer navigation technology assisted orbital fracture repair from Jan.2019 to Feb.2021 in the Affiliated Hospital of Yangzhou University were analyzed.There were 29 cases of blow-out fractures and 10 cases of orbital zygomatic fractures.The postoperative follow-up time was 6-12 months.The postoperative complication,the degree of anatomical reduction,diplopia and extraocular muscle movement limitation were observed.Results During the follow-up time,there were no serious complications such as optic nerve injury or vision loss,and the postoperative CT examination showed fine anatomical reduction of fracture.At 6 months after operation,the degree of diplopia significantly improved compared with that preoperatively(Z=-3.64,P<0.001).The degree of limitation of extraocular muscle movement also significantly improved compared with that preoperatively(Z=-3.31,P=0.001).The difference in bilateral enophthalmos decreased from[2.50(2.00,3.00)]mm before operation to[0.00(0.00,1.00)]mm at 1 week after operation(Z=-5.65,P<0.001);the difference in bilateral orbital volume before operation was(2.31±0.63)mm^(3),which reduced to(0.47±0.38)mm^(3) at 1 week after operation(t=16.82,P<0.001);the symmetry difference of the orbital zone in 10 patients with orbital zygomatic jaw fracture decreased from(8.72±1.05)mm before surgery to(1.20±0.49)mm at 1 week after surgery(t=23.39,P<0.001).Conclusion The application of 3D printing combined with computer navigation technology is effective for the reconstruction of orbital fractures.
