白内障术后眼内感染影响因素及泪液中细胞因子水平变化的意义探讨

摘　　要：目的探讨老年性白内障术后眼内感染影响因素及泪液中细胞因子水平变化的意义。方法回顾性病例对照研究。分析焦作煤业集团中央医院2017年5月至2021年4月老年性白内障手术100例(100眼)临床资料,其中术后眼内感染者50例(50眼)为观察组,术后无眼内感染者50例(50眼)为对照组。术后定量分析泪液中IL-2、IL-2/SIL-2R及IGF-1水平。结果观察组泪液中IL-2、IL-2/SIL-2R、IGF-1水平均低于对照组(t=5.95,3.45,9.49;均P<0.001),SIL-2R水平高于对照组(t=7.82,P=0.001);IL-2/SIL-2R(OR=0.53,95%CI=0.37~0.74,P<0.001)、IGF-1(OR=0.53,95%CI=0.37~0.74,P<0.001)为老年性白内障术后眼内感染独立保护因素;常规感染标志物血清PCT水平为(4.12±1.25)μg/L,hs-CRP水平为(24.41±7.19)mg/L,二者与泪液中IL-2/SIL-2R(r=-0.56,P<0.001;r=-0.62,P<0.001)、IGF-1(r=-0.60,P<0.001;r=-0.65,P<0.001)均呈负相关关系。结论泪液中IL-2、IL-2/SIL-2R、IGF-1的低表达与老年性白内障术后感染的发生具有相关性。Objective To investigate the influencing factors of intraocular infection in patients with senile cataract and significance of level changes of the interleukin-2(IL-2)/soluble IL-2 receptor(SIL-2R)and insulin-like growth factor-1(IGF-1)in tear fluid.Methods This was a retrospective case-control study.The clinical data of 100 eyes of 100 patients undergoing senile cataract surgery from May 2017 to Apr.2012 in the Central Hospital of Jiaozuo Coal Industry Group were retrospectively analyzed.Among them,50 patients with postoperative intraocular infection were included in the observation group,and 50 patients without postoperative intraocular infection were included in the control group.The levels of IL-2,IL-2/SIL-2R and IGF-1 in tears after operation were quantitatively analyzed.Results The levels of IL-2,IL-2/SIL-2R,and IGF-1 in the tears of the observation group were all lower than that of the control group,and the level of SIL-2R was higher than that of the control group(t=5.95,9.49,7.82,3.45;all P<0.001).Logistic regression analysis showed that IL-2/SIL-2R(OR=0.53,95%CI=0.37-0.74,P<0.001)and IGF-1(OR=0.53,95%CI=0.37-0.74,P<0.001)were independent protective factors for postoperative intraocular infection in patients with senile cataract;The serum PCT levels of routine infection markers was(4.12±1.25)μg/L,and the hs-CRP level was(24.41±7.19)mg/L.The PCT and hs-CRP both had a negative correlation with IL-2/SIL-2R(r=-0.56,P<0.001;r=-0.62,P<0.001)and IGF-1(r=-0.60,P<0.001;r=-0.65,P<0.001)in tears.Conclusion The low expression of IL-2/SIL-2R and IGF-1 in tears is correlated with the occurrence of intraocular infection after senile cataract surgery.
