摘 要:目的探讨肝癌患者CT灌注成像(CTPI)参数与黏蛋白1(MUC1)、存活蛋白(survivin)及微血管密度(MVD)的关系。方法选取73例原发性肝癌患者,均接受CTPI检查,取73例原发性肝癌患者的肝癌组织和至少15 mm处癌旁正常组织,采用免疫组化法检测MUC1、survivin的表达情况及MVD。CTPI灌注参数[总肝灌注量(TLP)、肝动脉灌注量(HAP)、肝动脉灌注指数(HAI)、门静脉灌注量(HPP)]与MUC1、survivin、MVD相关性采用Pearson相关分析。结果肝癌组织中HPP、TLP均明显低于癌旁正常组织(P﹤0.01),HAP、HAI均明显高于癌旁正常组织(P﹤0.01),MUC1、survivin阳性表达率和MVD均明显高于癌旁正常组织(P﹤0.01)。Pearson相关分析结果显示,HAP与survivin、MUC1、MVD均呈正相关(P﹤0.01);HPP、TLP与survivin、MUC1、MVD均无相关性(P﹥0.05);HAI与MVD呈正相关(P﹤0.01),但与MUC1、survivin均无相关性(P﹥0.05)。结论肝癌患者肝脏CTPI灌注参数与MUC1、survivin及MVD可能有关,可为肿瘤血管生成及相应生物学行为提供实时、无创的评价,为早期诊断、治疗方案拟订提供参考依据。Objective To investigate the relationship between CT-perfusion imaging(CTPI)parameters with mucin 1(MUC1),survivin and microvessel density(MVD)in patients with liver cancer.Method A total of 73 patients with primary liver cancer were enrolled,all patients received CTPI examination,liver cancer tissues and at least 15 mm adjacent normal tissues from 73 patients with primary liver cancer were collected,the expression of MUC1,survivin and MVD were detected by immunohistochemistry.The correlation between CTPI parameters[total liver perfusion(TLP),hepatic artery perfusion(HAP),hepatic artery perfusion index(HAI),hepatic portal vein perfusion(HPP)]and MUC1,survivin,and MVD were analyzed by Pearson correlation.Result The HPP and TLP in liver cancer tissue were significantly lower than those in the adjacent normal tissue(P<0.01),and the HAP and HAI were significantly higher than those in the adjacent normal tissue(P<0.01).The positive expression rates of MUC1,survivin and MVD in liver cancer tissues were significantly higher than those in adjacent normal tissues(P<0.01).The results of Pearson correlation analysis showed that HAP was positively correlated with survivin,MUC1 and MVD(P<0.01),HPP and TLP had no correlation with survivin,MUC1 and MVD(P>0.05);HAI was positively correlated with MVD(P<0.01),but had no correlation with MUC1 and survivin(P>0.05).Conclusion The liver CTPI parameters of patients with liver cancer have a certain degree of correlation with MUC1,survivin and MVD,which can provide real-time,non-invasive evaluation of tumor angiogenesis and corresponding biological behaviors and provide reference for early diagnosis and treatment.
