摘 要:目的探讨基于信息-知识-信念-行为(IKAP)模式的健康教育在胶质瘤术后化疗患者中的应用效果。方法采用随机数字表、单盲法将82例胶质瘤患者分为观察组和对照组,每组41例,对照组给予常规健康宣教,观察组在对照组基础上给予基于IKAP模式的健康教育干预。比较两组患者的心理状态[焦虑自评量表(SAS)和抑郁自评量表(SDS)]、自我管理能力[中文版癌症自我管理效能量表(SUPPH)]、癌因性疲乏程度[中文版癌症疲乏量表(CFS)]、生活质量[简明健康状况调查问卷(SF-36)]和并发症发生情况。结果干预后,两组患者的SDS、SAS、CFS量表各维度评分及总分均低于本组干预前(P﹤0.05),且观察组患者SDS、SAS、CFS量表各维度评分及总分均低于对照组(P﹤0.05)。干预后,两组患者SUPPH、SF-36量表各维度评分及总分均高于本组干预前(P﹤0.05),且观察组患者SUPPH、SF-36量表各维度评分及总分均高于对照组(P﹤0.05)。观察组干预期间并发症总发生率为19.51%,低于对照组患者的41.46%(P﹤0.05)。结论基于IKAP模式的健康教育可有效缓解胶质瘤化疗患者的焦虑抑郁情绪、提高自我管理能力,对改善患者的生活质量和癌因性疲乏有积极意义,可有效减少并发症的发生风险。Objective To explore the effect of health education based on information-knowledge-attitude-practice(IKAP)model in patients with glioma undergoing chemotherapy after surgery.Method A total of 82 cases of glioma patients were divided into observation group and control group by random number table and single-blind method,41 cases in each group.The control group was given routine health education,and the observation group was given the health education intervention based on the IKAP model on the basis of the control group.The psychological state[self-rating anxiety scale(SAS),self-rating depression scale(SDS)],self-management ability[strategies used by people to promote health(SUPPH)],degree of cancer-related fatigue[cancer fatigue scale(CFS)],quality of life[the MOS item-short form health survey(SF-36)]and the incidence of complications were compared between the two groups.Result After intervention,the scores of each dimension of SDS,SAS and CFS scale and the total score of the two groups of patients were lower than those before intervention(P<0.05),and the scores of each dimension and total score of SDS,SAS,and CFS in the observation group were lower than those in the control group(P<0.05).After the intervention,the scores of each dimension and total score of SUPPH and SF-36 scale in the two groups were higher than those before intervention(P<0.05),and the scores of each dimension and total score of SUPPH and SF-36 scale in the observation group were higher than those in the control group(P<0.05).The total incidence of complications during the intervention period in the observation group was 19.51%,which was lower than 41.46%in the control group(P<0.05).Conclusion Health education based on IKAP model can effectively relieve anxiety and depression of glioma patients undergoing chemotherapy,improve selfmanagement ability,and has positive significance in improving patients’quality of life and cancer-related fatigue,and can effectively reduce the risk of complications.
