大肠水疗在预防性肠造口患者排粪造影后的应用体会

摘　　要：为探讨大肠水疗在预防性肠造口患者排粪造影后的应用效果,选取本院行直肠癌前切除术后预防性肠造口患者1030例。所有患者均于排粪造影后行大肠水疗治疗,治疗前严格评估、查体、宣教;治疗中严格掌握操作要领及并发症应急处置方法,同时做好患者心理护理;治疗后密切观察随访。结果显示,所有患者均顺利完成大肠水疗,共6例因久病体质虚弱在治疗过程中出现虚脱昏厥的情况,及时予以对症治疗后好转。无吻合口漏、出血、造口旁疝及造口脱垂等不良事件发生。结果表明,预防性肠造口患者排粪造影后行大肠水疗安全可靠,可以减少钡剂残留。This study was to investigate the application efficacy of colonic hydrotherapy for patients undergoing prophylactic enterostomy after defecography,enrolled 1030 cases undergoing the stomy after anterior resection for rectal cancer in authors’ hospital who all will receive the hydrotherapy after defecography,before hydrotherapy,took rigid evaluation,physical exam.,and teaching to them;when intratreatment rigidly master operation notice and emergency management of complication,meanwhile took mental nursing on patietns;after treatment gave close follow-up.As results,all patients completed the hydrotherapy,only 6 cases had collapse and faint in treatment course due to their asthenic somaplasm induced by prolonged illness,then got improved through timely target treatment.No anastomotic leakage,bleeding,hernia near by stoma and stoma prolapse,etc unward events did not be found.Results show that implementing colonic hydrotherapy for patients undergoing prophylactic enterostomy after defecography is safe and reliable,also can reduce barium remnant.
