全髋关节置换术治疗股骨头颈部肿瘤及瘤样病变的效果观察

摘　　要：目的探讨全髋关节置换术治疗股骨头颈部肿瘤及瘤样病变的临床疗效。方法回顾性纳入郑州市骨科医院2015年1月至2020年12月收治的股骨头颈部肿瘤及瘤样病变患者50例。根据手术方式将纳入患者分为常规手术组和全髋关节置换术组,每组25例。比较两组术中出血量、手术时间、总住院时间、术后6个月髋关节功能恢复情况、术后生活质量及随访1年后的预后情况。结果两组术中出血量、手术时间比较差异未见统计学意义(P>0.05);常规手术组总住院时间[(21.86±4.07)d]长于全髋关节置换术组[(15.31±3.28)d],P<0.05。术后6个月,全髋关节置换术组髋关节功能恢复优良率(88.0%,22/25)高于常规手术组(60.0%,15/25),P<0.05。常规手术组术后躯体活动度、疼痛程度、情感表达、睡眠程度、精力、社会活动等诺丁汉健康调查问卷评分均高于全髋关节置换术组(P均<0.05)。术后随访1年,常规手术组和全髋关节置换术组病死率、肿瘤复发/转移率比较差异未见统计学意义(P>0.05)。结论全髋关节置换术治疗股骨头颈部肿瘤及瘤样病变具有较好的临床疗效,可以缩短术后住院时间,更好地恢复髋关节功能,有效缓解术后疼痛,提高患者生活质量,且安全性较高。Objective To investigate the clinical efficacy of total hip arthroplasty in the treatment of bone neoplasms and tumor-like lesions of femoral head and neck.Methods Fifty patients with bone neoplasms and tumor-like lesions of femoral head and neck admitted to Zhengzhou Orthopedics Hospital from January 2015 to December 2020 were included retrospectively.Patients were divided into routine operation group and total hip arthroplasty group according to surgery methods,with 25 cases in each group.The intraoperative blood loss,operation time,total hospital stay,hip function recovery 6 months after operation,postoperative quality of life,and prognosis during 1 year of follow-up were compared between the two groups.Results There was no significant difference in intraoperative blood loss and operation time between the two groups(P>0.05).The total hospital stay of routine operation group was(21.86±4.07)d,longer than the(15.31±3.28)d of total hip arthroplasty group(P<0.05).The excellent and good rate of hip joint function recovery in total hip arthroplasty group(88.0%,22/25)was significantly higher than that in routine operation group(60.0%,15/25)6 months after operation,P<0.05.The Nottingham health profile scores,including physical activity,pain degree,emotional expression,sleep degree,energy,and social activities of the routine operation group were significantly higher than those of the total hip arthroplasty group after operation(all P<0.05).There was no significant difference in case fatality rate and tumor recurrence/metastasis rate between the routine operation group and total hip arthroplasty group during 1 year of postoperative follow-up(P>0.05).Conclusions Total hip arthroplasty has a good clinical effect in the treatment of bone neoplasms and tumor-like lesions of femoral head and neck,which can shorten postoperative hospital stay and promote hip joint function recovery.It can also effectively relieve the postoperation pain and improve the quality of life of patients,with high safety.
