阿加曲班联合丁苯酞治疗缺血性脑卒中的临床效果

摘　　要：目的探究阿加曲班联合丁苯酞治疗缺血性脑卒中(IS)的临床疗效。方法抽取2020年3月至2021年3月在河南中医药大学人民医院接受诊治的120例IS患者,通过随机数字表法分为观察组(60例)与对照组(60例)。两组均给予常规对症治疗,对照组另给予阿加曲班治疗,观察组在对照组治疗基础上口服丁苯酞软胶囊。比较两组临床疗效、美国国立卫生研究院卒中量表(NIHSS)评分、Barthel指数评分、相关凝血指标及氧化应激指标变化。记录两组药物不良反应发生率。结果观察组临床总有效率(96.67%,58/60)高于对照组(83.33%,50/60),P<0.05。治疗后,观察组NIHSS评分低于对照组,Barthel指数评分高于对照组(P<0.05)。治疗后,观察组凝血酶原时间、纤维蛋白原、一氧化氮、丙二醛水平低于对照组,超氧化物歧化酶、过氧化氢酶高于对照组(P<0.05)。对照组(11.67%,7/60)与观察组(8.33%,5/60)药物不良反应发生率比较差异未见统计学意义(P>0.05)。结论阿加曲班联合丁苯酞治疗可有效修复IS患者神经功能损伤,优化凝血功能,减轻机体内部相关应激反应,改善其日常生活能力,具有良好安全性。Objective To investigate the effects of argatroban combined with butylphthalide on patients with ischemic stroke(IS).Methods A total of 120 IS patients treated in People’s Hospital of Henan University of Traditional Chinese Medicine from March 2020 to March 2021 were selected and divided into observation group(60 cases)and control group(60 cases)by random number table method.Both groups were given routine symptomatic treatment;the control group was given argatroban treatment;and the observation group orally taken butylphthalide soft capsule based on the treatment of the control group.The clinical efficacy,the National Institutes of Health Stroke Scale(NIHSS)score,and Barthel index scores,related coagulation indicators,and oxidative stress indicators were compared between the two groups.And adverse reactions of the two groups were recorded.Results The total effective rate in the observation group(96.67%,58/60)was significantly higher than that in the control group(83.33%,50/60),P<0.05.After treatment,the NIHSS score of observation group was lower than that of control group,and Barthel index score of observation group was higher than that of control group(P<0.05).The prothrombin time,fibrinogen,nitric oxygen,and malondialdehyde of the observation group were significantly lower than those of the control group(P<0.05).The superoxide dismutase and catalase levels of the observation group were significantly higher than those of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the control group(11.67%,7/60)and observation group(8.33%,5/60),P>0.05.Conclusions Argatroban combined with butylphthalide therapy can effectively repair the nerve function damage of patients with IS,optimize blood coagulation function,reduce internal stress response of the body,and improve ability of daily living,with high safety.
