摘 要:目的:探讨脑干听觉诱发电位(BAEP)对基底节和放射冠区无症状脑梗死的诊断价值。方法:选择菏泽市第三人民医院2016年8月至2020年12月由磁共振成像确诊的基底节和放射冠区无症状脑梗死患者117例,其中放射冠区梗死35例作为A组,基底节梗死14例作为B组,放射冠伴有基底节梗死68例作为C组。使用Nicolet Viking Quest诱发电位肌电图仪检测BAEP,分析各组峰潜伏期(PL)、峰间潜伏期(IPL)及波幅等参数。结果:与对照组比较,患者组波Ⅰ、波Ⅴ的PL延长、波Ⅲ-Ⅴ及波Ⅰ-Ⅴ的IPL延长(P<0.05)。与对照组比较,MRA正常组和异常组波Ⅴ的PL、波Ⅲ-Ⅴ及波Ⅰ-Ⅴ的IPL延长(P<0.05)。与对照组、A、B组比较,C组波Ⅲ的PL延长(P<0.05);与对照组比较,A组和C组波Ⅴ的PL、波Ⅲ-Ⅴ及Ⅰ-Ⅴ的IPL的延长(P<0.05);与A组比较,C组波Ⅲ、波Ⅴ的PL延长(P<0.05)。患者组波幅低于对照组(P<0.05)。结论:BAEP检测是一项客观敏感的神经电生理学方法,对于评价基底节和放射冠区无症状脑梗死对脑干功能的影响具有重要价值。Objective:To investigate the diagnostic value of brainstem auditory evoked potential(BAEP)in patients with silent brain infarction(SBI)in coronal radiata and basal ganglia.Methods:A total of 117 patients with SBI diagnosed by magnetic resonance imaging from August 2016 to December 2020 in Heze Third People's Hospital were enrolled in the study,including 35 patients with coronal radiata infarction as group A,14 patients with basal ganglia infarction as group B,and 68 patients with coronal radiata and basal ganglia infarction as group C.BAEP was performed in all patients.The peak latency(PL),inter peak latency(IPL)and amplitude were analyzed.Results:Compared with the control group,the PL of waveⅠand waveⅤ,IPL of waveⅢ-ⅤandⅠ-Ⅴwere prolonged in the patient groups(P<0.05).Compared with the control group,the PL of wave V,IPL of wave Ⅲ-V and wave I-V were prolonged in normal and abnormal MRA groups(P<0.05).Compared with the control group,group A and group B,the PL of waveⅢwas prolonged in group C(P<0.05).Compared with the control group,the PL of wave V,IPL of wave Ⅲ-V and I-V were prolonged in group A and group C(P<0.05).Compared with group A,the PL of wave Ⅲ and wave V was prolonged in group C(P<0.05).The amplitude in the patient groups was lower than that in the control group(P<0.05).Conclusion:BAEP detection is an objective and sensitive neuroelectrophysiological method,which is of great value in evaluating the effects of SBI in basal ganglia and coronal radiata on brain stem function.
