一清片联合替硝唑片治疗代谢相关脂肪性肝病伴牙周炎临床研究

摘　　要：目的:观察一清片联合替硝唑片治疗代谢相关脂肪性肝病(MAFLD)伴牙周炎对患者高迁移率族蛋白B1(HMGB1)、高迁移率族蛋白A1(HMGA1)及肝脏代谢的影响。方法:选取95例MAFLD伴慢性牙周炎患者作为MAFLD伴牙周炎组,100例慢性牙周炎患者作为牙周炎组,100例正常体检人群作为健康组。MAFLD伴牙周炎组和牙周炎组口服一清片联合替硝唑片治疗4周。比较MAFLD伴牙周炎组、牙周炎组治疗前后的菌斑指数(PLI)、龈沟出血指数(SBI)、临床附着丧失(CAL)、牙周探诊深度(PD)及甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C),并采用Western Blot检测3组HMGA1、HMGB1的蛋白相对表达量。结果:与健康组治疗前比较,MAFLD伴牙周炎组与牙周炎组治疗前HMGB1、HMGA1蛋白相对表达量均升高(P<0.05)。与同组治疗前比较,MAFLD伴牙周炎组与牙周炎组治疗后PLI、SBI、LDL-C、TC、TG水平及HMGB1、HMGA1蛋白相对表达量均降低(P<0.05),CAL、PD均变浅(P<0.05);与MAFLD伴牙周炎组治疗前比较,牙周炎组治疗前PLI、SBI及HMGB1、HMGA1蛋白相对表达量均较低(P<0.05),CAL、PD均较浅(P<0.05)。牙周炎组治疗前LDL-C、TC、TG水平与MAFLD伴牙周炎组治疗前比较,差异无统计学意义(P>0.05);牙周炎组治疗后LDL-C、TC、TG水平与MAFLD伴牙周炎组治疗后比较,差异无统计学意义(P>0.05)。结论:一清片联合替硝唑片治疗MAFLD伴牙周炎患者明显改善了患者牙周指标,同时有效降低了MAFLD的炎症反应,疗效显著。Objective:To observe the effect of the therapy of Yiqing tablets combined with tinidazole tablets on high mobility group B1(HMGB1)proteins,high mobility group A1(HMGA1)proteins and liver metabolism in patients with metabolic associated fatty liver disease(MAFLD)complicated with periodontitis.Methods:A total of 95 MAFLD patients complicated with chronic periodontitis were selected as the MAFLD complicated with periodontitis group,100 patients with chronic periodontitis as the periodontitis group,and 100 people with normal physical examination results as the healthy group.The MAFLD complicated with periodontitis group and the periodontitis group were treated with oral administration of Yiqing tablets combined with tinidazole tablets for four weeks.Before and after treatment,the Plaque Index(PLI),Sulcus Bleeding Index(SBI),Clinical Attachment Loss(CAL),periodontal probing depth(PD),triglyceride(TG),total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)in the MAFLD complicated with periodontitis group and the periodontitis group were compared.The relative expressions of HMGA1 and HMGB1 proteins in the three groups were detected by Western Blot.Results:Before treatment,the relative expressions of HMGB1 and HMGA1 proteins in the MAFLD complicated with periodontitis group and the periodontitis group were increased when compared with those in the healthy group(P<0.05).After treatment,the levels of PLI,SBI,LDL-C,TC and TG and the relative expressions of HMGB1 and HMGA1 proteins in the MAFLD complicated with periodontitis group and the periodontitis group were decreased when compared with those before treatment(P<0.05),and CAL and PD became shallower(P<0.05);before treatment,the levels of PLI and SBI and relative expressions of HMGB1 and HMGA1 proteins in the periodontitis group were lower than those in the MAFLD complicated with periodontitis group(P<0.05),and CAL and PD were shallower(P<0.05).Before treatment,there was no significant difference being found in the comparison of levels of LDL-C,TC and TG in th
