摘 要:目的比较牙源性颌骨囊肿术后自发性骨再生与骨填充材料Bio-Oss移植后骨再生的效果。方法选取2019年10月-2021年3月安徽医科大学附属安庆医院口外病房收治的50例牙源性颌骨囊肿患者(最大直径≤20 mm),随机分为观察组和对照组,各25例。对照组囊肿摘除后以自身血凝块充填愈合,观察组术后填充Bio-Oss,比较两组伤口一般情况,囊肿摘除后1、3、6个月的骨再生率、骨愈合情况及颌骨外形恢复情况。结果两组术后伤口感染、伤口裂开、下唇麻木发生率比较,差异无统计学意义(P>0.05);术后1、3、6个月,观察组骨缺损体积小于对照组,差异有统计学意义(P<0.05);观察组骨再生率高于对照组,差异有统计学意义(P<0.05);观察组骨CT值大于对照组,差异有统计学意义(P<0.05);观察组骨愈合指数高于对照组,差异有统计学意义(P<0.05)。术后6个月,观察组外形恢复较好,对照组5例外形恢复欠佳,出现软组织塌陷。结论对于直径≤20 mm的牙源性颌骨囊肿,术后使用Bio-Oss骨粉移植后的骨再生效果优于自身血凝块充填的自发性骨再生。Objective To compare the effect of spontaneous bone regeneration after odontogenic jaw cyst and bone filling material Bio-Oss transplantation.Methods A total of 50 patients with odontogenic jaw cysts(maximum diameter≤20 mm)admitted to the Department of Otolaryngology,Anqing Hospital,Anhui Medical University from October 2019 to March 2021 were selected and randomly divided into the observation group and the control group,with 25 cases in each group.The cysts in the control group were healed with their own blood clots after removal,and the observation group was filled with Bio-Oss after operation.The general wound conditions,bone regeneration rate,bone healing and jaw shape recovery after 1,3 and 6 months of cyst removal were compared between the two groups.Results There was no significant difference in the incidence of postoperative wound infection,wound dehiscence and lower lip numbness between the two groups(P>0.05).At 1,3 and 6 months after operation,the bone defect volume in the observation group was smaller than that in the control group,and the difference was statistically significant(P<0.05).The bone regeneration rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).At 1,3 and 6 months after operation,the bone CT value of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05);the bone healing index of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05);the appearance of the observation group was better,and 5 cases in the control group had poor shape recovery and soft tissue collapse.Conclusion For odontogenic jaw cysts with diameter≤20 mm,the effect of bone regeneration after Bio-Oss bone powder transplantation is better than that of spontaneous bone regeneration after autologous blood clot filling.
