单双手提下颌在松动牙齿缺齿老年患者经口可视硬性光棒气管插管中的应用

摘　　要：目的探讨单手提下颌和双手提下颌在松动牙齿缺齿老年患者经口可视硬性光棒气管插管中的临床应用效果。方法选择宿迁市第一人民医院2020年9月至2021年6月有松动牙齿缺齿需行全身麻醉经口气管插管术老年患者60例,随机均分为两组:单手提下颌组(A组)30例和双手提下颌组(B组)30例,比较两组患者气管插管成功率、光棒位置调整次数、气管插管时间和气管插管后并发症。结果A组和B组患者气管插管成功率均为100%,B组光棒位置调整次数(1.7±0.4)次,少于A组的(2.8±0.7)次,差异有统计学意义(P<0.05),B组气管插管操作时间(16.36±5.23)s,短于A组的(27.65±8.81)s,差异有统计学意义(P<0.05),插管并发症分别为A组咽喉黏膜损伤1例、术后咽喉疼痛3例、声音嘶哑1例;B组术后咽喉疼痛1例,B组气管插管并发症低于A组,差异有统计学意义(P<0.05)。结论单手提下颌和双手提下颌法经口可视硬性光棒气管插管均可安全应用于松动牙齿缺齿的老年患者,而双手提下颌法插管时间短和并发症少等优点。Objective To investigate the clinical application effect of single and two-handed jaw-thrust in the oral video light wand endotracheal intubation in elderly patients with loose teeth and missing teeth.Methods Sixty patients with loose teeth and missing teeth who needed general anesthesia and oral endotracheal intubation from September2020 to June 2021 in Suqian First People’s Hospital were randomly divided into two groups:single-handed jaw-thrust group(group A,n=30)and two-handed jaw-thrust group(group B,n=30).The success rate of endotracheal intubation,light wand position adjustment times,endotracheal intubation time,and complications after endotracheal intubation between the two groups were compared.Results The success rate of tracheal intubation in both group was 100%.The number of light wand position adjustments in group B was(1.7±0.4),which was less than(2.8±0.7)in group A,and the difference was statistically significant(P<0.05).The operation time of endotracheal intubation in group B was(16.36±5.23)s,which was shorter than(27.65±8.81)s of group A,and the difference was statistically significant(P<0.05).The complications of intubation were throat mucosal injury in 1 case,postoperative throat pain in 3 cases,and hoarseness in1 case in group A.There was 1 case of postoperative throat pain in group B.The complications of endotracheal intubation in group B were lower than those in group A,and the difference had statistical significance(P<0.05).Conclusion Both single-handed and two-handed jaw-thrust in the oral video light wand endotracheal intubation can be safely used in elderly patients with loose teeth and missing teeth.The two-handed jaw-thrust has the advantages of short endotracheal intubation time and fewer complications.
