摘　　要：目的:探讨带蒂背阔肌kiss皮瓣修复在局部晚期乳腺癌术后胸壁缺损中的应用效果。方法:回顾性分析2015年12月-2018年12月中山市人民医院收治的60例局部晚期乳腺癌患者的临床资料,患者于术中切除原发灶后均接受了胸壁缺损修复治疗,根据胸壁缺损修复方式的不同将患者分为试验组和对照组,每组30例。试验组采用带蒂背阔肌kiss皮瓣修复,对照组采用单纯背阔肌皮瓣转移修复。比较两组手术时间、切口愈合时间、并发症发生情况和复发率及生活质量评分(SF-36量表)。结果:试验组手术时间、切口愈合时间均短于对照组,差异均有统计学意义(P<0.05)。试验组并发症发生率和2年复发率均低于对照组,差异均有统计学意义(P<0.05)。术后6个月,试验组生活质量各项目评分均高于对照组,差异均有统计学意义(P<0.05)。结论:带蒂背阔肌kiss皮瓣修复在局部晚期乳腺癌术后胸壁缺损中效果确切,安全性高,术后恢复快,可有效降低复发风险。Objective:To investigate the effect of pedicled latissimus dorsi kiss flap repair chest wall defect after locally advanced breast cancer surgery.Method:The clinical data of 60 patients with locally advanced breast cancer admitted to Zhongshan People’s Hospital from December 2015 to December 2018 were retrospective analyzed.All patients received chest wall defect repair treatment after intraoperative resection of the primary lesion,according to the different repair methods of chest wall defect,patients were divided into experimental group and control group,30 cases in each group.The experimental group was treated with pedicled latissimus dorsi kiss flap repair,and the control group was treated with simple latissimus dorsi musculocutaneous flap transfer repair.The operation time,incision healing time,incidence of complications and recurrence rate,and quality of life score (SF-36 scale) were compared between two groups.Result:The operative time and wound healing time of the experimental group were shorter than those of the control group,the differences were statistically significant (P<0.05).The incidence of complications and 2-year recurrence of the experimental group were lower than those of the control group,the differences were statistically significant (P<0.05).6 months after surgery,all dimensions scores of life quality of the experimental group were higher than those of the control group,the differences were statistically significant (P<0.05).Conclusion:The pedicled latissimus dorsi kiss flap repair is effective in the repair of chest wall defect after locally advanced breast cancer surgery,with high safety,quick postoperative recovery and effective reduction of recurrence risk.
