β-磷酸三钙/壳聚糖水凝胶在牙髓根尖周病患者中的应用及对MMP-3表达的影响

摘　　要：目的:探讨β-磷酸三钙/壳聚糖水凝胶在牙髓根尖周病患者中的应用及对基质金属蛋白酶-3(MMP-3)表达的影响。方法:选取2018年1月-2021年1月朝阳市第二医院收治的60例牙髓根尖周病患者,将其依照1∶1的比例随机分为观察组与对照组,每组30例。所有患者均行根管填充治疗,对照组患者应用氢氧化钙糊剂进行处理,观察组患者应用β-磷酸三钙/壳聚糖水凝胶进行处理。比较两组患者的临床治疗效果,比较两组患者的治疗费用、治疗时间、疼痛消退时间、炎症消退时间和两组患者治疗前后炎症因子变化、MMP-3水平,以及不同时间的VAS评分。结果:观察组治疗总有效率为93.33%,高于对照组的73.33%,差异有统计学意义(P<0.05)。两组治疗费用比较,差异无统计学意义(P>0.05);观察组治疗时间、疼痛消退时间以及炎症消退时间均明显短于对照组,差异均有统计学意义(P<0.05)。治疗前,两组患者IL-6、IL-1、TNF-α、MMP-3水平比较,差异均无统计学意义(P>0.05);治疗后,观察组IL-6、IL-1、TNF-α、MMP-3水平均明显低于对照组,差异均有统计学意义(P<0.05)。治疗前,两组疼痛评分比较,差异无统计学意义(P>0.05);观察组患者治疗后2周、1个月、3个月的疼痛评分均低于对照组,差异均有统计学意义(P<0.05)。结论:在牙髓根尖周病患者的治疗过程中应用β-磷酸三钙/壳聚糖水凝胶能够减轻临床症状,提升治疗效果,与传统治疗费用无明显差异,减轻患者炎症反应与疼痛感,促进患者早日康复,值得临床应用推广。Objective:To explore the application of β-tricalcium phosphate/chitosan hydrogel in patients with pulp and periapical disease and its effect on matrix metalloproteinase-3 (MMP-3) expression.Method:A total of 60 patients with pulp and periapical disease admitted to the Second Hospital of Chaoyang City from January 2018 to January 2021 were randomly divided into observation group and control group according to the ratio of 1∶1,with 30 cases in each group.All patients were treated with root canal filling,the control group was treated with Calcium Hydroxide Paste,and the observation group was treated with β-tricalcium phosphate/chitosan hydrogel.The clinical treatment effects of the two groups were compared,and the treatment cost,treatment time,pain regression time,inflammation regression time,the changes of inflammatory factors,MMP-3 level and VAS score of the two groups before and after treatment were compared.Result:The total effective rate of observation group was 93.33%,higher than 73.33% of control group,the difference was statistically significant (P<0.05).There was no significant difference in treatment cost between the two groups (P>0.05);the treatment time,pain resolution time and inflammation resolution time in the observation group were significantly shorter than those in the control group,the differences were statistically significant (P<0.05).Before treatment,there were no significant differences in the levels of IL-6,IL-1,TNF-α and MMP-3 between two groups (P>0.05);after treatment,the levels of IL-6,IL-1,TNF-α and MMP-3 in the observation group were significantly lower than those in the control group,the differences were statistically significant (P<0.05).Before treatment,there was no significant difference in pain scores between the two groups (P>0.05);the pain scores of the observation group were lower than that of the control group at 2 weeks,1 month and 3 months after treatment,the differences were statistically significant (P<0.05).Conclusion:The application of β-tricalcium phosphate/chitos
