摘  要:目的 分析食管癌患者原发灶^(18)F-氟脱氧葡萄(^(18)F-FDG)正电子发射计算机断层成像(PET)/CT代谢参数与淋巴结转移的相关性。方法 选择食管癌患者83例。术前14 d内行18F-FDG PET/CT检查,测定计算原发灶的最大标准摄取值(SUVmax)、平均标准摄取值(SUVmean)、代谢体积(MTV)、总病变糖酵解(TLG)。以术后病理结果为标准,采用相关性分析及受试者工作特征(ROC)曲线评估各参数对淋巴结转移的诊断价值。结果 代谢参数中SUVmax和SUVmean与N分期无相关性(P>0.05),MTV、TLG与N分期均呈正相关(r=0.357、0.423,均P<0.05)。SUVmax、SUVmean、MTV、TLG的曲线下面积(AUC)分别为0.441、0.477、0.672、0.726,TLG的灵敏度为0.72,特异度为0.57。结论 食管癌原发灶18F-FDG PET/CT代谢参数MTV、TLG与食管癌淋巴结转移存在正相关,且TLG对淋巴结转移的预测价值高于MTV。Objective To analyze the correlation between ^(18)F-FDG PET/CT metabolic parameters and lymph node metastasis in patients with esophageal cancer.Methods Eighty-three patients with esophageal cancer were selected,^(18)F-FDG PET/CT was performed within 14 days before operation.SUVmax,SUVmean,MTV and TLG were measured and calculated.The postoperative pathological results were taken as the standard,correlation analysis and receiver operating characteristic(ROC) curve were used to evaluate the diagnostic value of each parameter for lymph node metastasis.Results SUVmax and SUVmean were not correlated with N stage(P>0.05).MTV and TLG were positively correlated with N stage,and the correlation coefficients were 0.357 and 0.423 respectively(P<0.05).The AUC of SUVmax,SUVmean,MTV and TLG were 0.441,0.477,0.672 and 0.726.The sensitivity and specificity of TLG were 0.72 and 0.57,respectively.Conclusions ^(18)F-FDG PET/CT metabolic parameters MTV and TLG are positively correlated with lymph node metastasis of esophageal cancer.The predictive value of TLG for lymph node metastasis is higher than that of MTV.
