摘 要:目的分析重组人生长激素在促进颅咽管瘤术后患儿生长发育中的应用效果。方法回顾性收集2019年1月至2020年12月我院收治的经左旋甲状腺素泼尼松、去氨加压素治疗的67例颅咽管瘤术后患儿临床资料作为对照组,另选择同期收治的67例联合重组人生长激素治疗的颅咽管瘤术后患儿临床资料作为观察组,2组治疗时间均为6个月。比较2组治疗前和治疗6个月后生长发育情况、甲状腺功能、肝肾功能及糖脂代谢指标。结果与治疗前比较,治疗6个月后2组身高均升高,观察组高于对照组(P<0.05)。与治疗前比较,治疗6个月后观察组生长速率(GV)、身高标准差(HtSDS)、胰岛素样生长因子(IGF)-1、类胰岛素样生长因子结合蛋白-3(IGFBP3)水平均升高,且高于对照组(P<0.05)。与治疗前比较,治疗6个月后2组血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)、丙氨酸转氨酶(ALT)、门冬氨酸转氨酶(AST)、肌酐(SCr)、糖化血红蛋白(HbA1c)、空腹血糖(FPG)、高密度脂肪酸胆固醇(HDL-C)、低密度脂肪酸胆固醇(LDL-C)、总胆固醇(TC)、甘油三酯(TG)水平变化差异均无统计学意义(P>0.05),且组间比较,差异无统计学意义(P>0.05)。结论重组人生长激素可有效促进颅咽管瘤术后患儿生长发育,同时不会对患儿甲状腺功能、肝肾功能及糖脂代谢造成较大影响,具有较好的疗效和安全性。Objective To analyze the application effect of recombinant human growth hormone in promoting the growth and development of postoperative craniopharyngioma children.Methods From January 2019 to December 2020,the clinical data of 67 postoperative children with craniopharyngioma,who were treated with levothyroxine prednisone and desmopressin,were collected retrospectively as the control group,and the clinical data of 67 postoperative children with craniopharyngioma,who were treated with recombinant human growth hormone in the same period,were selected as the observation group.Chilidren in both groups were treated for 6 months.The growth and development,thyroid function,liver and kidney function and glucose and lipid metabolism indexes of the two groups were compared before treatment and after 6 months of treatment.Results After 6 months of treatment,the height of both groups increased,and the children in observation group was higher than the ones in control group(P<0.05).Compared with before treatment,after 6 months of treatment,the growth rate(GV),height standard deviation(HtSDS),levels of serum insulin-like growth factor-1(IGF-1)and insulin-like growth factor-binding protein-3(IGFBP3)in the observation group increased,and higher than those of control group(P<0.05).Compared with before treatment,after 6 months of treatment,the levels of serum free triiodothyronine(FT3),free thyroxine(FT4),thyroid stimulating hormone(TSH),alanine aminotransferase(ALT),aspartate aminotransferase(AST),creatinine(SCR),glycosylated hemoglobin(HbA1c),fasting blood glucose(FPG),high density fatty acid cholesterol(HDL-C),low density fatty acid cholesterol(LDL-C),total cholesterol(TC)and triglyceride(TG)in the two groups were no significant difference(P>0.05).Conclusion Recombant human growth hormone could effectively promote the growth and development of children after craniopharyngioma surgery,and had no significant effect on thyroid function,liver and kidney function and glucose and lipid metabolism in children,which is with better effic
