重度面神经炎患者联合应用糖皮质激素和复方甘露醇治疗的效果

摘　　要：目的:探讨重度面神经炎患者联合应用糖皮质激素和复方甘露醇治疗的效果。方法:选择2020年2月-2021年2月抚顺市中医院收治的重度面神经炎患者82例,随机分为两组,每组41例。对照组应用糖皮质激素,研究组在对照组基础上应用复方甘露醇。比较两组治疗前后面神经功能缺损、面神经功能分级、肌电图改变情况及生活质量评分。结果:研究组治疗7、14 d后Portmann评分均高于对照组(P<0.05);研究组治疗后Ⅰ、Ⅱ级例数均多于对照组,Ⅲ、Ⅳ级例数均少于对照组(P<0.05);研究组治疗后R1潜伏期短于对照组,CAMP波幅低于对照组(P<0.05);治疗后,两组MH、RP、SF、BP评分较治疗前均升高,且研究组均高于对照组(P<0.05)。结论:重度面神经炎患者相较于单独应用糖皮质激素,联合应用糖皮质激素+复方甘露醇治疗效果更佳,可以有效改善面神经缺损情况,降低面神经功能分级,改善肌电图情况,提高生活质量。Objective:To explore the effect of combined treatment with Glucocorticoid and Compound Mannitol in patients with severe facial neuritis.Method:A total of 82 patients with severe facial neuritis in Fushun Hospital of Chinese Medicine from February 2020 to February 2021 were selected and randomly divided into two groups,41 cases in each group.The control group was treated with Glucocorticoid,and the study group was treated with Compound Mannitol on the basis of the control group.The neurological deficits,facial nerve function classification,electromyogram changes and quality of life scores in two groups before and after treatment were compared.Result:The Portmann scores in the study group after 7 and 14 d of treatment were higher than those in the control group(P<0.05).After treatment,the number of gradeⅠandⅡcases in the study group were more than those in the control group,and the number of gradeⅢandⅣcases were less than those in the control group(P<0.05).The incubation period of R1 in the study group was shorter than that in the control group,and the CAMP amplitude was lower than that in the control group(P<0.05);after treatment,the scores of MH,RP,SF and BP in the study group were higher than those in the control group(P<0.05).Conclusion:Compared with Glucocorticoid alone,the treatment effect of Glucocorticoid+Compound Mannitol in patients with severe facial neuritis is better,it can effectively improve the facial nerve defect,reduce the facial nerve function classification,and improve the patient’s electromyogram,improve their quality of life.
