摘 要:目的:探讨前庭功能训练对脑卒中患者平衡功能以及下肢三维运动学参数的影响。方法:以2019年2月-2021年1月在潍坊市中医院住院治疗的84例脑卒中患者为研究对象,根据电脑随机分配的方式分为对照组和研究组,各42例。两组患者病情稳定后均给予常规的康复训练,研究组在常规康复训练的基础上增加前庭功能训练。连续训练2个月后比较两组的临床疗效;用简化的Fugl-Meyer运动评估量表(FMA)、Berg平衡量表(BBS)、Barthel指数(BI)和Gait watch三维步态分析仪比较两组训练前后的下肢运动功能、平衡功能、日常自理能力以及下肢关节运动参数。结果:研究组总有效率高于对照组(P<0.05);研究组和对照组患者的FMA、BBS和BI评分均高于康复训练前(P<0.05),且研究组均高于对照组(P<0.05);研究组和对照组训练后患侧髋膝关节的最大伸展度及最大屈曲度受限程度均明显改善(P<0.05),患侧步长均显著提高,支撑相缩短且摆动相均延长(P<0.05)。研究组下肢三维运动学参数改善程度均优于对照组(P<0.05)。结论:前庭功能训练联合常规康复治疗可有效提高脑卒中患者平衡能力、下肢运动和日常自理能力,改善患者下肢三维步态参数,提高康复效果。Objective:To investigate the effect of vestibular function training on balance function and three dimensional kinematics parameters of lower extremities in stroke patients.Method:A total of 84 patients with stroke who were hospitalized in Weifang Hospital of Traditional Chinese Medicine from February 2019 to January 2021 were selected as the research subjects.According to the computer randomly assigned method,they were divided into control group and research group,with 42 cases in each group.Patients of the two groups were given routine rehabilitation training after the condition was stable,while the research group added vestibular function training on the basis of routine rehabilitation training.The clinical effects of the two groups were compared after 2 months of continuous training.The simplified Fugl-Meyer motor assessment scale(FMA),Berg balance scale(BBS),Barthel index(BI)and Gait watch 3D Gait analyzer were used to compare the lower limb motor function,balance function,daily self-care ability and lower limb joint motion parameters before and after training in the two groups.Result:The total effective rate of the research group was higher than that of the control group(P<0.05).The scores of FMA,BBS and BI in the research group and the control group were higher than those before rehabilitation training(P<0.05),and those in the research group were higher than those in the control group(P<0.05).In the research group and the control group,the maximum extension and limitation of maximum flexion of the affected hip and knee were significantly improved after training(P<0.05),the step length of the affected side were significantly increased,the support phase were shortened and the swing phase were prolonged(P<0.05).The improvement of lower limb 3D kinematics parameters in the research group were better than those in the control group(P<0.05).Conclusion:The vestibular function training combined with routine rehabilitation treatment can effectively improve the balance ability,lower limb movement and daily self-care
