摘　　要：目的对妇科内镜手术培训与考核体系(gynecological endoscopic surgical education and assessment,GESEA)在妇产科住院医师规范化培训(简称住陪)中进行了中期测评,评价GESEA在试点单位中的应用有效性及培训次数的价值。方法纳入2018-2019年进入上海交通大学医学院附属仁济医院妇产科住院医师规范化培训基地的26名学员。将来自欧洲的成熟的GESEA应用于学员培训,该体系具有完备、细致的训练流程和可信度较高的考核方式。本研究通过进行中期测评,比较26名学员入门测试与中期测试成绩变化,以及比较不同培训次数之间成绩差异,结果采用SPSS 26.0软件进行统计分析。结果比较入门测试与中期测试成绩,显示所有模块培训后优秀(excellent)百分比均明显上升(P<0.05),所有模块入门测试与中期测试成绩差异均有统计学意义(P<0.001);对中期测试成绩分组比较,培训次数>4次的学员的腹腔镜模块(LASTT,SUTT)的中期测试成绩明显高于培训次数≤4次的学员(P<0.05)。培训次数>4次的学员的宫腔镜模块(HYSTT)的中期测试成绩虽高于培训次数≤4次的学员,但组间差异无统计学意义(P>0.05)。结论融入GESEA能够有效提高并规范住院医师规范化培训学员的内镜手术基础操作技能,培训前后成绩有显著差异,不同培训次数之间成绩有差异,有待完成规定次数。Objective To evaluate the application of Gynaecological Endoscopic Surgical Education and Assessment(GESEA)in the standardized residency program for residents of gynaecology and obstetrics and the relationship between effectiveness and training frequency.Methods 26 trainees who entered the standardized residency program of gynaecology and obstetrics and in Renji Hospital affiliated to Shanghai Jiao Tong University School of Medicine from 2018 to 2019 were included.The mature GESEA from Europe is applied to the managing of trainees.The system has a complete and detailed training process and a highly reliable assessment method.In this study,through the mid-term evaluation,we compared the scores of 26 trainees in the entrance test and the mid-term test,and compared the differences through different training frequency.The results were analyzed by SPSS 26 software.Results Comparing the scores of entry test and mid-term test,it showed that the percentage of excellent in all modules increased significantly after training(P<0.05),and the difference between the scores of entry test and mid-term test was statistically significant(P<0.001);By grouping and comparing the mid-term test results,the mid-term test results of laparoscopic module(LASTT,SUTT)of students with training times>4 times were significantly higher than those of students with training times≤4 times(P<0.05).Although the mid-term test scores of hysteroscopy module(HYSTT)of students with training times>4 were higher than those of students with training times≤4,there was no significant difference between groups(P>0.05).Conclusion The integration of GESEA can effectively improve and standardize the basic operating skills of endoscopic surgery for trainees in the standardized training base for resident physicians.There are significant differences in performance before and after training,and there are differences in performance between different training frequency.
