超声乳化联合人工晶体置入术治疗高度近视合并白内障的疗效和可行性

摘　　要：目的探讨高度近视合并白内障患者采用超声乳化联合人工晶体置入术的疗效和可行性。方法选取2018年10月至2020年9月本院收治的157例(256眼)高度近视合并白内障患者作为研究对象,所有患者均采用超声乳化联合人工晶体置入术治疗,比较手术前后患者裸眼视力、最佳矫正视力、视觉质量、屈光度及并发症发生情况。结果术后1、3、6个月,患者裸眼视力和最佳矫正视力均明显优于术前(P<0.05)。术后3个月,患者视觉质量明显好转,术后不同时期患者的屈光度均明显优于术前,呈现逐渐好转趋势(P<0.05)。术后有2眼发生后囊膜破裂,1眼发生悬韧带断裂,角膜水肿18眼,高压眼3例,经过妥善处理后情况均好转。随访6个月,未发生其他并发症。结论高度近视合并白内障患者采用超声乳化联合人工晶体置入术的治疗效果较好,且并发症较少,值得临床推广应用。Objective To investigate the efficacy and feasibility of phacoemulsification combined with intraocular lens implantation in high myopia patients with cataract.Methods 157 cases(256 eyes)of high myopia patients with cataract from October 2018 to September 2020 were selected as the research subjects.All patients were treated with phacoemulsification combined with intraocular lens implantation.The uncorrected visual acuity,best corrected visual acuity,visual quality,diopter and complications were compared before and after operation.Results 1,3 and 6 months after operation,the uncorrected visual acuity and best corrected visual acuity were significantly better than those before operation(P<0.05).3 months after operation,the visual quality of the patients improved significantly,and the diopter of the patients in different periods after operation was significantly better than that before operation,showing a trend of gradual improvement(P<0.05).After operation,posterior capsule rupture occurred in 2eyes,suspensory ligament rupture occurred in 1 eye,corneal edema occurred in 18 eyes,and hypertension occurred in 3 eyes.Follow up for 6 months showed no other complications.Conclusion Phacoemulsification combined with intraocular lens implantation is effective in the treatment of high myopia with cataract,with fewer complications,which is worthy of clinical application.
