硅水凝胶角膜接触镜senofilcon A和comfilcon A在LASEK术后应用的对比研究

摘　　要：目的对比硅水凝胶角膜接触镜senofilcon A和comfilcon A在准分子激光上皮下角膜磨镶术(LASEK)术后应用的临床效果。方法采用前瞻性随机双盲自身对照试验的方法,纳入行LASEK患者39例(78眼),术后同一患者一眼配戴senofilcon A角膜接触镜(A组),另一眼配戴comfilcon A角膜接触镜(B组),连续配戴7 d。评估并比较两组患眼术后主观感受和客观体征的差别。结果手术当天,B组患眼的眼痛评分为(2.03±2.03)分,高于A组[(1.49±1.68)分](P=0.024)。术后1 d, B组患眼的眼痛、流泪、异物感评分均高于A组(P;=0.001,P;=0.032,P;=0.025);术后3 d、7 d,两组患眼间的主观感受评分差异均无统计学意义(均为P>0.05)。在术后视力、角膜上皮愈合、角膜上皮水肿、结膜充血等客观体征方面,两组患眼在观察期内差异均无统计学意义(均为P>0.05)。结论硅水凝胶角膜接触镜senofilcon A和comfilcon A在LASEK术后促进角膜上皮再生方面没有明显差别,但senofilcon A在术后早期患者舒适度方面较comfilcon A更有优势。Objective To evaluate and compare the clinical effect of senofilcon A and comfilcon A silicone hydrogel contact lenses following the laser-assisted subepithelial keratectomy(LASEK).Methods A prospective, randomized, double-blind, self-control trial was conducted in 39 patients(78 eyes) who underwent LASEK. After the operation, one eye wore senofilcon A lens(group A) and the fellow eye wore comfilcon A lens(group B) for 7 consecutive days. The subjective feelings and objective signs between the two groups were evaluated and compared.Results On the day of surgery, the pain score of eyes in group B was 2.03±2.03, higher than that in group A(1.49±1.68)(P=0.024). On the first day after surgery, the scores for eye pain, lacrimation and foreign body sensation in group B were higher than those in group A(P;=0.001, P;=0.032, P;=0.025). On the third and seventh days after surgery, there was no significant difference in subjective feelings between the two groups(all P>0.05). During the observation period, there was no significant difference between the two groups in objective signs, such as visual acuity, corneal epithelial healing, corneal epithelial edema, and conjunctival congestion(all P>0.05).Conclusion Senofilcon A and comfilcon A silicone hydrogel contact lenses have no significant difference in promoting corneal epithelial regeneration after LASEK. However, senofilcon A lenses are more comfortable than comfilcon A lenses in the early postoperative period.
