超声引导下射频消融联合负压吸脂微创治疗皮下脂肪瘤疗效观察

摘　　要：目的:探讨超声引导下射频消融联合负压吸脂微创治疗皮下脂肪瘤的临床疗效。方法:回顾性分析2019年9月至2021年1月郑州大学第一附属医院整形外科收治的45例皮下脂肪瘤患者的临床资料,按照治疗方法分为射频消融组(n=20)和常规手术组(n=25)。比较两组患者的术中出血量、住院时间、伤口愈合时间、瘢痕长度、视觉模拟评分(VAS)得分及美容满意度得分的差异;观察两组患者并发症及复发情况。结果:射频消融组患者的术中出血量、住院时间、伤口愈合时间以及瘢痕长度、VAS得分均低于常规手术组,美容效果满意度得分高于常规手术组(P<0.05)。与常规手术组相比,射频消融组患者出现的不良反应较轻微、短暂,均未发生发热、伤口感染、皮肤坏死等并发症。术后随访3~20个月,所有患者均未出现原发病灶复发。结论:超声引导下射频消融联合负压吸脂微创治疗皮下脂肪瘤具有安全性高、并发症少、恢复快、美观、患者满意度较高等优点,适用于相对注重外观的皮下脂肪瘤患者,是一种较好的微创治疗方法。Aim:To determine the clinical effectiveness of ultrasound-guided radiofrequency ablation coupled with negative pressure liposuction in the treatment of subcutaneous lipoma.Methods:The clinical data of 45 patients with subcutaneous lipoma who were hospitalized to the Department of Plastic Surgery at the First Affiliated Hospital of Zhengzhou University from September 2019 to January 2021 were retrospectively analyzed.According to the treatment approaches,they were allocated into the radiofrequency ablation group(n=20)and routine open surgery group(n=25).The intraoperative blood loss,length of hospital stay,wound healing time,scar length,visual analog scale(VAS)score and cosmetic satisfaction score were compared between the two groups.Moreover,complications and recurrences were observed.Results:The amount of intraoperative blood loss,length of hospital stay,wound healing time,scar length and VAS score for patients in the radiofrequency ablation group were lower than that those in routine open surgery group,and the satisfaction score of cosmetic effect was higher than that of the routine open surgery group(P<0.05).Patients who had radiofrequency ablation experienced fewer temporary adverse effects than those who underwent routine open surgery.They also had fewer cases of wound infection or skin necrosis,and no recurrence of the primary lesions was observed in all patients during a follow-up of 3-20 months.Conclusion:Ultrasound-guided radiofrequency ablation in conjunction with negative pressure liposuction for minimally invasive treatment of subcutaneous lipoma has the advantages such as higher safety,fewer complications,swift recovery,a more aesthetically pleasing appearance,and elevated patients′satisfaction rate.It is suitable for individuals with subcutaneous lipoma who are more concerned with their looks,and it is also a more effective and minimally invasive treatment option.
