皮肤镜辅助诊断小汗腺汗孔瘤1例

摘　　要：报告1例小汗腺汗孔瘤。患者男,58岁。左足跟部红色肿物10余年。皮肤科检查:左足跟腱部可见直径约1 cm的红黑色隆起性结节,质软,边界清楚,伴外周褐黑色色素沉着。皮肤镜下可见边界清晰的类圆形结节,皮损中央可见多个叶状血管,血管周围有白色晕。皮损组织病理检查:真皮内可见与表皮相连的肿瘤细胞团块,向下延展进入真皮,瘤细胞大小较一致,部分呈立方形,有深嗜碱性圆形胞核。诊断:小汗腺汗孔瘤。予手术切除,随访半年无复发。A 58-year-old man presented with a nodule on his left heel for more than ten years.A soft demarcated dark red nodule,sized 1 cm x 1cm,was found on the left heel with peripheral brownish pigmentation.Dermoscopic examination showed a well-circumscribed dome-shaped nodule with centralized leaf-like vessels,surrounded by whitish halos.Pathological findings included protrusion of tumor mass from the epidermis into the dermis.The tumor cells were uniform in size,some of which were cuboidal,with dark basophilic round nuclei.The diagnosis of eccrine poroma,dermal duct tumor type,was confirmed.The lesion was completely excised,without recurrence.
